Let's Dance Studio
Extended Liability Waiver and Release of Claims
By acknowledging this waiver electronically, I, the undersigned parent or legal guardian of the minor child(ren) enrolled in classes at Let's Dance (“the Studio”), hereby agree to the following terms on behalf of myself and my child(ren):
1. Assumption of Risk.
I understand and acknowledge that participation in dance classes, rehearsals, performances, and related activities involves inherent risks of physical injury, including but not limited to sprains, strains, broken bones, concussions, and in rare cases, more serious injuries. I knowingly and voluntarily assume all such risks associated with participation, whether occurring on or off the studio premises, and regardless of who is at fault.
2. Waiver and Release of Liability.
To the fullest extent permitted by law, I, on behalf of myself, my child(ren), and our respective heirs, assigns, and legal representatives, hereby waive, release, and discharge Let's Dance, its owners, officers, directors, instructors, employees, agents, volunteers, and any affiliated entities (collectively, “Released Parties”) from any and all liability, claims, demands, or causes of action arising out of or related to participation in any program, class, event, or activity sponsored by or associated with the Studio.
This waiver includes claims for personal injury, property damage, medical expenses, and wrongful death, whether arising from the negligence of the Released Parties or otherwise.
3. Indemnification.
I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, liabilities, damages, losses, costs, and expenses (including attorney’s fees) resulting from participation in any studio-related activity, or from any breach of this agreement by me or my child.
4. Medical Authorization.
In the event of an injury or medical emergency involving my child, I authorize the Studio and its personnel to obtain medical treatment for my child and to act on my behalf in securing such treatment. I agree to assume full financial responsibility for any medical services rendered.
5. COVID-19 and Infectious Disease Assumption of Risk.
I understand that despite safety protocols, there is a risk of exposure to COVID-19 or other contagious illnesses when participating in group activities. I acknowledge this risk and voluntarily agree to allow my child(ren) to participate. I release and hold harmless the Released Parties from any liability related to COVID-19 exposure or illness.
6. Photo and Media Release.
I grant the Studio permission to photograph or video record my child for use in promotional materials, including online and social media platforms. I understand that no names will be used without additional consent. (If you wish to opt out, you must notify the Studio in writing.)
7. Acknowledgment and Understanding.
I acknowledge that I have read this Liability Waiver and Release of Claims in its entirety. I understand its contents and agree to its terms voluntarily and without coercion. I certify that I am the legal parent or guardian of the child(ren) named on this account and that I have the authority to sign on their behalf.
