Privacy Policy Agreement
Storm Dance Academy is committed to protecting the privacy of our families. By creating an account and enrolling in programs, I (the parent/guardian and/or adult participant) acknowledge and agree to the following:
1. Information Collection
Storm Dance Academy collects personal information including names, contact details, and payment information for the purpose of registration, communication, and billing.
2. Use of Information
Information is used solely for studio operations, including class registration, scheduling, communication, and payment processing.
3. Data Protection
Storm Dance Academy uses secure systems, including third-party software, to store and process information. Payment information is processed securely and is not stored directly by the studio.
4. Information Sharing
Storm Dance Academy does not sell or share personal information with outside parties, except as necessary to operate the studio (such as payment processing or studio management software).
5. Communication
By providing contact information, I consent to receive communication from Storm Dance Academy regarding classes, schedules, events, and important updates.
6. Agreement & Acknowledgment

STORM DANCE ACADEMY
Code of Conduct (Dancers & Parents)
At Storm Dance Academy, we are committed to creating a positive, respectful, and focused environment where all dancers can grow. By enrolling, I (the parent/guardian and/or adult participant) agree to uphold the following expectations:

1. Respect for Staff & Students
All dancers and parents are expected to treat instructors, staff, and fellow students with respect at all times. Disrespectful language, negative behavior, or disruptive actions will not be tolerated.
2. Classroom Behavior (Dancers)
Dancers are expected to:
* Listen and follow instructor directions
*Stay engaged and participate fully
*Refrain from disruptive behavior
*Demonstrate teamwork and a positive attitude
Repeated disruptions may result in removal from class.
3. Parent Conduct & Boundaries
Parents are expected to:
* Allow instructors to manage the classroom without interruption
* Address questions or concerns respectfully and at appropriate times
* Refrain from coaching, correcting, or distracting dancers during class
* Support studio policies and staff decisions
4. Viewing & Studio Etiquette
To maintain a focused learning environment:
* Parents may only observe classes when permitted by the studio
* Lobby and waiting areas should remain respectful and quiet
* Siblings must be supervised at all times
5. Communication Expectations
All communication with staff must remain respectful and professional. Concerns should be directed to studio leadership rather than discussed negatively with other parents or students.
6. Social Media Conduct
Families are expected to represent Storm Dance Academy positively online. Negative, harmful, or inappropriate posts regarding the studio, staff, or dancers may result in dismissal from the program.
7. Attendance & Commitment
Regular attendance is expected. Excessive absences, lateness, or lack of commitment may impact a dancer’s progress and participation opportunities.
8. Dress Code & Preparedness
Dancers must arrive on time, properly dressed, and prepared for class. This includes appropriate attire, shoes, and hair as required by the studio.
9. Studio Authority
Storm Dance Academy reserves the right to dismiss any dancer or family who does not adhere to the Code of Conduct or disrupts the studio environment. No refunds will be issued in the event of dismissal.
10. Agreement & Acknowledgment

STORM DANCE ACADEMY
Liability Waiver & Release of Claims
By enrolling in classes, camps, rehearsals, performances, or any activities associated with Storm Dance Academy, I (the parent/guardian and/or adult participant) acknowledge and agree to the following:

1. Assumption of Risk
I understand that dance and related physical activities involve inherent risks, including but not limited to physical injury, falls, strains, and other unforeseen risks. I voluntarily choose to participate (or allow my child to participate) in these activities with full knowledge of these risks.
2. Release of Liability
I hereby release, waive, and discharge Storm Dance Academy, its owners, directors, instructors, staff, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained while participating in studio activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.
3. Medical Authorization
In the event of an injury or medical emergency, I authorize Storm Dance Academy staff to seek medical treatment for myself/my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred.
4. Physical Fitness & Participation
I certify that I (or my child) am physically able to participate in dance activities. I agree to inform the studio of any medical conditions, injuries, or limitations that may affect participation.
5. Communicable Illness Acknowledgment
I understand that participation in group activities carries a risk of exposure to illness. I agree not to attend classes if the dancer is experiencing symptoms of illness.
6. Personal Property
Storm Dance Academy is not responsible for lost, stolen, or damaged personal belongings.
7. Agreement & Acknowledgment
I have read this waiver carefully and fully understand its terms. By signing below (or accepting electronically), I acknowledge that I am giving up certain legal rights, including the right to sue.
I agree that this waiver applies to all current and future participation at Storm Dance Academy and must be renewed annually.

Photo & Video Release Agreement
By enrolling in classes, camps, rehearsals, performances, or any activities associated with Storm Dance Academy, I (the parent/guardian and/or adult participant) acknowledge and agree to the following:

1. Permission to Capture Media
I grant permission for Storm Dance Academy to photograph and/or record video of myself/my child during classes, rehearsals, performances, and studio events.
2. Use of Media
I authorize Storm Dance Academy to use these photos and videos for promotional and marketing purposes, including but not limited to:
* Social media (Instagram, Facebook, etc.)
* Studio website
* Advertising and promotional materials
* Printed materials (brochures, flyers, posters)
3. Ownership of Media
I understand that all photos and videos taken by Storm Dance Academy are the property of the studio and may be used without further approval.
4. No Compensation
I acknowledge that I will not receive compensation for the use of these images or videos.
5. Respectful Use
Storm Dance Academy agrees to use all media in a respectful and appropriate manner.
6. Opt-Out Option
I understand that if I prefer that my child not be photographed or recorded, I must notify Storm Dance Academy in writing. I acknowledge that while the studio will make reasonable efforts to honor this request, it cannot guarantee exclusion from all group images, performances, or event recordings.
7. Agreement & Acknowledgment

STORM DANCE ACADEMY
Tuition & Payment Policy Agreement
By enrolling in classes, camps, or programs at Storm Dance Academy, I (the parent/guardian and/or adult participant) acknowledge and agree to the following financial policies:

1. Tuition Commitment
Enrollment at Storm Dance Academy is a commitment for the full dance season (September through May), regardless of attendance. Tuition is based on enrollment, not class attendance.
2. Payment Method & Auto-Pay
A valid credit/debit card must be kept on file. By enrolling, I authorize Storm Dance Academy to process automatic monthly payments for all tuition and fees.
3. Monthly Tuition Payments
Tuition is charged in monthly installments and is due on the 1st of each month.
4. Late Payments
If a payment is declined or not received, a $25 late fee will be applied if payment is not successfully processed within 3 days. Accounts with outstanding balances may result in the dancer being unable to participate in class until the account is current.
5. Non-Refundable Payments
All tuition, registration fees, and payments made to Storm Dance Academy are non-refundable and non-transferable, including missed classes, withdrawals, or schedule changes.
6. Withdrawal Policy
A written notice of withdrawal is required at least 30 days in advance. Tuition will continue to be charged during the notice period. Verbal notice or non-attendance does not constitute withdrawal.
7. Missed Classes
No refunds or credits will be issued for missed classes. Make-up classes may be offered at the studio’s discretion.
8. Additional Fees
I understand that additional fees may apply throughout the season, including but not limited to:
* Registration fees
* Costume fees
* Recital/performance fees
* Competition fees (if applicable)
All additional fees are subject to their own timelines and are non-refundable once processed.
9. Collections & Failed Payments
If a payment is declined, I authorize Storm Dance Academy to reprocess the payment. Repeated failed payments may incur additional fees. Accounts that become significantly past due may be sent to collections.
10. Agreement & Acknowledgment
I have read and understand this Tuition & Payment Policy. By signing below (or accepting electronically), I agree to abide by all terms outlined above.
