I/We (The dancer and parent/guardian) hereby assume the risks of participating in any classes, activities, or events run by The Dancer’s Corner LLC (dba Storytellers Dance Co). I understand that dance lessons and their associated activities could result in personal injury, property damage, and other risks that may arise from negligence or carelessness on the part of the persons or entities being released.

I certify that the releasee is in good health and was not advised to not participate in this activity by a medical professional.

In signing this form, I/We hereby waive, release and discharge The Dancer’s Corner LLC and its teachers, students, staff, volunteers, and facilities used from any and all liability and responsibility for incidents including but not limited to personal injuries, disability, property damage, property theft, death, or accidents that occur before, during, or after this activity or its events.

I/We agree to hold harmless and promise not to sue the entities and persons mentioned in this waiver from all liabilities or claims made as a result of participation in this activity and its associated events.

I CERTIFY THAT I HAVE READ THIS DOCUMENT AND FULLY UNDERSTAND ITS CONTENT. I UNDERSTAND THAT THIS IS A RELESE OF LIABILITY CONTRACT, AND I AM SIGNING IT OF MY OWN FREE WILL.

PARENT/GUARDIAN WAIVER FOR MINORS (for all students under 18 years old): The undersigned parent or legal guardian confirms that they have sufficient permission to register their child/ward for this activity and events. Additionally, the undersigned parent/guardian certifies that they have consented to their child or ward’s participation in this activity or event and have agreed individually and on behalf of the child or ward to the terms of the accident waiver and release of liability set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties.

I hereby grant permission for The Dancer's Corner LLC, doing business as Storytellers Dance Co., to use my child(ren)'s/my likeness in photos and/or videos in any web-based media (website, online ads), social media (studio Instagram and Facebook Pages), marketing materials (including but not limited to flyers, posters, or brochures), or other publication, display, or broadcast. I understand that my child(ren)'s first name may be used along with these photos and videos.

I further understand that I am consenting to Storytellers Dance Co. editing any photos or videos in whole or in part to fit the business's purposes.

I understand that I do not have any right to royalties, commission, or any other payment related to the use of these photos and videos by Storytellers Dance Co.

I understand that I can revoke my consent to use photos/videos of myself/my child(ren) at any time by submitting a written request to Storytellers Dane Co. and that doing so will not affect my/my child(ren)'s participation in Storytellers Dance Co. programming. Furthermore, I understand that, while no additional content will be shared following the removal of consent, materials that have already been published will remain published.

I understand that I am not required to sign this consent form and that the choice not to do so will not affect my/my child(ren)'s participating in the dance studio's programming.
