In the event of a medical emergency, I authorize Moscow Dance Company staff to seek emergency medical treatment for myself or my child. I understand that every effort will be made to contact me immediately. I assume full financial responsibility for any medical treatment or transport deemed necessary.

I certify that I have disclosed any known medical conditions, allergies, or limitations that could affect participation.

Moscow Dance Company – Photo/Video Release

I hereby grant permission to Moscow Dance Company to photograph and/or video record myself or my child during classes, performances, or events for promotional and marketing purposes, including but not limited to use on the Studio’s website, social media, and printed materials.

Moscow Dance Company – Liability Waiver and Assumption of Risk

I, the undersigned, acknowledge that participation in dance classes and related activities at Moscow Dance Company (the “Studio”) involves inherent risks, including but not limited to falls, injuries, or accidents.

I voluntarily assume all risks associated with participation for myself and/or my minor child and agree to release, indemnify, and hold harmless Moscow Dance Company, its owners, instructors, employees, and agents from any and all claims or liability for personal injury, property damage, or loss arising from participation in any Studio activities, whether on-site or off-site (e.g., performances, events, or rehearsals).

I understand that it is my responsibility to consult a physician prior to participation and to inform the Studio of any physical limitations, conditions, or concerns.
