I declare that the aforesaid participant is in good health, with no physical conditions that might prevent their participation in strenuous and/or rigorous dance activities and other training and performances connected with dance and/or exercise. I understand that dance and exercise activities have inherent risks of injury. Being fully aware of all risks, I consent to have my child or myself participate in classes, rehearsals, performances, recitals or other activities offered by Creative Edge Dance Studio Inc. I, on behalf of myself, my heirs, executors, agents and representatives, hereby indemnify, release and forever hold harmless Creative Edge Dance Inc., as well as its directors, employees and instructors, from any and all claims of liability arising from any accident, personal injury, death, or property loss or damage sustained by my child or myself, while that person is involved in activities connected to Creative Edge Dance Studio Inc., including classes, rehearsals, performances, etc. I understand that at times, for proper instruction and safety, physical contact is required and necessary. I acknowledge that because of the physical nature of dance, there will be physical contact between instructors, staff and students.

Fees and Scheduling

The first months payment is due at time of registration and is non-refundable in order to hold a place for your dancer. Monthly tuition fees for your dancer’s lessons are due on the 1st of each month September 2026 - June 2027. Tuition fees are charged in three instalments #1 Due at Registration, #2 November 1st 2026, and #3 December 15th 2026 and do not increase or decrease during shorter or longer months. Payments can be made by credit card, debit, e-transfer or cheque. All payments made after the 15th of each month are subject to a $15.00 late charge (unless otherwise arranged). A Recital Fee of $165.00 (includes HST) applies per student, per class and includes one costume per class as well as the development and production of either an online or in-person year-end recital. The recital fee does not include theatre tickets or personal items such as shoes, tights, make-up, hair products etc. Recital fees are 100% non-refundable. The Recital fee for the year-end Recital is due at the time of registration and is non-refundable. $45 Registration Fee per family due at registration. I understand that Creative Edge Dance Studio does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc.

Withdrawal Policy

If your dancer wishes to not continue dancing, the studio must be advised in writing 30 days prior and pay the remaining months lessons. Non-attendance is not accepted as a cancellation notice from any class. If costumes have already been ordered, I understand that I am responsible for full payment of all costumes for all my dancer(s).

GENERAL RELEASE FOR ALL PROGRAM PARTICIPANTS

On behalf of myself, my heirs, my persona representatives or assigns, or as the legal guardian of the minor registrant, do hereby release, indemnify and save harmless, waive and forever discharge Creative Edge Dance Studio Inc, its directors, employees, volunteers from liability for any and all claims resulting in personal injury, accidents or illness (including death) and property loss arising from, but not limited to participation in activities, classes, observation and use of facilities, premises or equipment. I also authorize Creative Edge Dance Studio Inc to obtain any medical care deemed necessary in the event of an injury and agree to pay for any resulting medical expenses. By signing this form, I acknowledge having read, understood and agreed to this waiver, releases and indemnity. I hereby grant my permission for my child to participate in all Creative Edge Dance Studio Inc program activities including photographs, recordings and public performances and allow the use of any such material in which my child appears, for promotional, educational or commercial purposes.
