By signing below, I confirm that I have read, reviewed with my student(s), understood, and agree to follow the
policies and procedures outlined in the Revival Dance Lab Dancer Handbook that can be found under the resources tab at revivaldancelab.com

I understand that this includes, but is not limited to:
-Expectations
-Code of Conduct
-Dress Code
-Performance Opportunities and policies
-Tuition Policies

I recognize that these policies are in place to support a safe, respectful, and
organized learning environment for all dancers, families, and staff.

I understand that failure to follow the policies may result in communication from
studio leadership and, if necessary, further action to ensure the safety and success
of all students.

I understand that I can request a physical copy of this handbook from RDL admin.

In consideration of participation in dance classes, rehearsals, performances, and other
activities provided by Revival Dance Lab, LLC, I agree to the following:

1. Acknowledgment of Risk
I understand that participation in dance and physical activity involves inherent risks,
including but not limited to muscle strain, sprains, falls, collisions, broken bones, and
other injuries. These risks exist regardless of supervision, instruction, or experience. I
knowingly and voluntarily accept all such risks, whether known or unknown.

2. Waiver and Release of Liability
I hereby release and hold harmless Revival Dance Lab, LLC, its owners, instructors,
staff, independent contractors, volunteers, and affiliates from any and all liability,
claims, or causes of action for injury, illness, property damage, or other loss that may
result from participation in studio activities, whether on or off studio premises,
including travel. This release applies to all causes, including negligence, to the fullest
extent permitted by law.

3. Responsibility Outside of Class Time
I understand that Revival Dance Lab, LLC is not responsible for supervising dancers
before or after their scheduled class times. I agree to ensure timely drop-off and
pick-up for my dancer.

4. Media Disclaimer
By enrolling your child and/or participating in our programs, you grant Revival Dance Lab, LLC permission to use your or your Childs likeness unless you provide a written request to opt out. If you do not wish for you or your child to appear in media materials, please inform the studio in writing prior to participation.

I have read and understand this waiver and release.

I acknowledge that by signing below, I am waiving certain legal rights, including the right to sue Revival Dance Lab,
LLC or its representatives for injuries or damages.

I certify that I am the participant’s
parent or legal guardian (if under 18), or that I am 18 years of age or older and signing on my own behalf.
