LIABILITY WAIVER
I understand and agree that dance and physical activities involve inherent risks of injury. I certify that I/my child is physically able to participate in the classes and activities provided by Sol 66 Dance Academy. I understand that Sol 66 Dance Academy, its owners, staff, and instructors are not responsible for any injuries or illnesses sustained while participating in studio classes, rehearsals, events, or performances.

I voluntarily assume all risks related to participation and hereby release and discharge Sol 66 Dance Academy, its employees, instructors, and representatives from any and all claims of liability arising out of or connected to participation in studio activities.

MEDICAL EMERGENCY CONSENT
In the event of a medical emergency, I authorize Sol 66 Dance Academy staff to obtain emergency medical treatment for my child or myself (if over 18) if I cannot be reached. I assume full responsibility for any medical expenses incurred.
