The Inspired Arts Company Liability Waiver & Assumption of Risk I, the undersigned, understand that participation in dance classes and activities at The Inspired Arts Company involves physical activity and the risk of injury. I voluntarily assume all risks related to participation in any and all programs offered by the studio. I hereby release, waive, and discharge The Inspired Arts Company, its owners (including Morgan Jezierski), instructors, employees, and volunteers from any and all liability for injuries, damages, or losses incurred during participation, whether caused by negligence or otherwise. I certify that I (or my child) am physically able to participate in dance activities and have no known medical conditions that would prevent safe participation.
The Inspired Arts Company Medical Release In the event of a medical emergency, I authorize The Inspired Arts Company and its staff to seek appropriate medical attention and care for me or my child. I understand that every effort will be made to contact me in such an event. I agree to be financially responsible for any medical treatment provided.
The Inspired Arts Company Media Release I grant permission to The Inspired Arts Company to photograph, record, and use images and videos of me or my child for promotional, advertising, or social media purposes. I understand that these images may be used without further notice or compensation.
The Inspired Arts Company Policies Agreement I acknowledge that I have received and read The Inspired Arts Company's studio policies, including class conduct, tuition and payment, attendance, dress code, and withdrawal procedures. I agree to follow these policies as a condition of participation.
