I confirm that I am over the age of 18 and the parent or legal guardian of the child(ren) I am registering with Inspire Dance Academy. I understand and agree that I am financially responsible for all tuition, fees, and charges associated with their enrollment and participation.

I have received or will obtain a copy of Inspire Dance Academy LLC policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Inspire Dance Academy LLC does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, COVID-19, weather, etc. Refunds are also not issued for tuition, costumes, performance fees, recital fees, media, photos or other items specifically ordered for your dancer. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Inspire Dance Academy LLC, and I voluntarily assume the risks associated with such participation.

I authorize Inspire Dance Academy LLC to use any images and videos including my image for promotional or advertising purposes. I authorize Inspire Dance Academy, LLC and its officers, legal representatives and affiliates to publish, edit, modify and create images for artistic representation, publicity and advertising.

I further release Inspire Dance Academy, LLC from any legal action, claims and demands arising from images taken. I grant sole ownership of all images obtained to Inspire Dance Academy, LLC and will not sell or publish any images without the expressed written consent of Inspire Dance Academy, LLC. I acknowledge that I am at least 18 years of age and can legally sign this document

I, the undersigned parent or legal guardian of the participant named above, hereby acknowledge that participation in dance classes, rehearsals, performances, and related activities at Inspire Dance Academy involves inherent risks, including but not limited to the risk of personal injury, property damage, or death.

I voluntarily assume all such risks associated with participation, whether known or unknown, and on behalf of myself and my child, I hereby waive, release, discharge, and hold harmless Inspire Dance Academy, its owners, instructors, employees, volunteers, and agents from any and all claims, liabilities, damages, or costs (including attorney’s fees) arising out of or in connection with any personal injury, illness, accident, or property damage incurred by the participant during or in connection with participation in any Inspire Dance Academy activity.
