Waiver of Liability (required to take classes)
As a participant in any program or class of DANCE LA I recognize and acknowledge the nature of the activities I and/or my child will be participating in and that the participant is in the proper physical condition and capable of participating in the related activities, understanding that DANCE LA is not in any way responsible for making such a determination.
I agree to waive and relinquish all claims that I or my minor child may have as a result of participating in any program or class against DANCE LA and its officers, agents, servants, employees and independent contractors. I further agree to indemnify and hold harmless and defend DANCE LA and its officers, agents, servants, employees and independent contractors from my claims resulting from injuries including death, damages and losses sustained by me or my minor child that arise out of, in connection with, or in any way associated with the activities of this program.
Photograph release
I hereby permit the use of my, or my child’s image to be photographed, videotaped or otherwise recorded for use in DANCE LA’s publicity or educational materials. These materials include, but are not limited to, photographs of classes and performances, newsletters, various other publications, DANCE LA’s internet webpage and social media outlets and videos of classes or performances.
