I acknowledge that participation in dance classes involves a risk of injury. I voluntarily accept this risk and release Rise Dance Co. from liability for injuries sustained during participation.

I understand that tuition, registration fees, and other fees are non-refundable upon withdrawal unless otherwise stated by Rise Dance Co.

I grant permission for photographs and videos of my child to be used by Rise Dance Co. for promotional purposes, including social media, website content, advertising, and studio publications. (Please reach out if otherwise)
