I acknowledge and agree to the following:
Assumption of Risk: I understand that participation in dance, tumbling, and related physical activities carries a risk of injury. I voluntarily assume all risk associated with my child’s participation at Dance Republic.
Medical Treatment: In the event of an emergency, I authorize Dance Republic staff to obtain emergency medical treatment for my child. I understand that any medical expenses incurred will be my responsibility.
Release of Liability: I release and hold harmless Dance Republic, its staff, directors, contractors, and affiliated parties from any and all claims, liabilities, or causes of action arising from injury, illness, or property damage that may occur while participating in any studio activity or program.
Media Release: I give permission for Dance Republic to use photos and videos of my child taken during classes, camps, and events for marketing purposes including social media, print materials, and the studio website. Dancer names will never be used publicly in connection with any photos or videos.
Behavior Expectations: I agree to support the studio’s policies and procedures and understand that Dance Republic reserves the right to dismiss a student for disruptive or unsafe behavior without refund.
