By signing up with First State Academy of Dance, you agree that there are no drops accepted after December 31st when tuition will continue until June 5th. I also understand that all students (except for Open Ballet and Teen/Adult Ballet classes) are required to purchase a costume and participate in the end of the year Dance Concert.

By signing up for the school year session, I understand there are no drops accepted after December 31 and that tuition and costume fees will still be automatically withdrawn until June 5th, whether or not my child(ren) attend classes.

I hereby allow my child’s or my picture and/or video to be taken for advertising and promotion reasons. Pictures/Videos may appear on websites, YouTube, Twitter, Instagram, Facebook, other social media, brochures, flyers, programs, or newspapers with or without their name. Pictures/videos will become the property of the DanceCompany-X, Inc. Company and used at any time & without permission.

I understand that I or my child am dancing, rehearsing, and performing with First State Academy of Dance, Inc. and I agree to hold harmless the rental facilities & owner/director, Michele Xiques, and any rehearsal/choreographers /dance instructors for any incidental injury that may occur as a result thereof, such as sprains, stress fractures, broken bones, etc., and I agree that I will take no action against the owner and/or any rehearsal assistants of the foregoing company and permit them to obtain any necessary emergency treatments including 911 transport to the local emergency room that I will be financially responsible for with the understanding of being notified as soon as possible.
