Elizabeth Mission of Dance is registered with the State of Florida as a Ballroom Dance Studio, Registration No. ______.
By registering for classes or events with Elizabeth Mission of Dance, you (the “Student” or “Parent/Guardian”) agree to the following terms, which together form a binding contract:
Services & Costs:
You are enrolling in dance classes (ballet, tap, jazz, or other programs) at the rates shown in our class schedule. Your total payment obligation will not exceed $250 per month (or per session). The hourly rate for instruction is $85.
Contract Term:
This agreement is valid from the date of registration through the end of the month/session in which you are enrolled. Renewals are processed monthly or per session. No contract will exceed 36 months.
Cancellation Rights:
You may cancel this agreement without penalty or obligation by delivering written notice within 3 business days (excluding weekends and holidays) of signing. Notice must be delivered in person or by certified mail to Elizabeth Mission of Dance.
Refund Policy:
I understand that all tuition and registration fees are non-refundable unless the class is cancelled by Elizabeth Mission of Dance. Missed classes due to personal reasons (including illness or travel) will not be refunded or credited.
Liability Release
I agree to hold Elizabeth Mission of Dance harmless and free from any liability, financial or otherwise, for injuries incurred during participation, regardless of whether such injuries are caused by negligence. I understand and accept the inherent risks associated with dance and physical activity and will make every effort to follow all safety precautions, whether written or verbally communicated.
Photo & Video Release
I consent to Elizabeth Mission of Dance photographing and/or recording myself and/or my child during classes and events. I understand that these images and recordings may be used for marketing and promotional purposes, including but not limited to print publications, the official website, and social media platforms.
Personal photography or videography is permitted; however, if another parent/guardian requests that their child not be photographed or recorded, I agree to respect their wishes and will refrain from capturing images or recordings of that child. In such cases, I will also delete any media inadvertently captured.
Class Cancellation
I understand that Elizabeth Mission of Dance reserves the right to cancel any class or program that does not meet the minimum enrollment requirements. In such cases, I will be notified via phone or email and will receive a full refund for any canceled classes.
Communication Agreement
I agree to receive communications from Elizabeth Mission of Dance via email, phone, or text regarding class schedules, updates, announcements, and other relevant information.
Medical Emergency Authorization
I authorize Elizabeth Mission of Dance staff to seek emergency medical treatment for myself or my child in the event of an injury or illness. I understand that I am responsible for all related medical expenses.
Behavior Expectations
I understand that students and parents are expected to demonstrate respectful and appropriate behavior at all times. Elizabeth Mission of Dance reserves the right to dismiss any student or parent whose behavior is disruptive or unsafe, with no refund issued.
Personal Property
Elizabeth Mission of Dance is not responsible for lost, stolen, or damaged personal items brought to classes or events.
Illness Policy
I agree not to send my child (or attend myself) if experiencing symptoms of illness. I understand that students showing signs of illness may be asked to leave class in order to maintain a safe and healthy environment.
Public Health Acknowledgment
I acknowledge the potential risk of exposure to contagious illnesses (such as COVID-19) and assume responsibility for that risk. I agree to comply with any health guidelines or safety protocols set by Elizabeth Mission of Dance.
