By registering my child(ren) with the Mid Mo Kats, I agree to participate (or allow my child(ren) and family members to participate) in the Mid Mo Kats, and hereby release Mid Mo Kats, its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child(ren) and family members) while participating in the Mid Mo Kats program, including travel to and from training sessions, recitals, competitions, conventions, or other scheduled organization activities.

I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) participating in the Mid Mo Kats program and other classes.

I certify that I am the parent or legal guardian for my child(ren). I hereby give my permission for any supervisor, coach or other team administrator associated with the Mid Mo Kats to seek and give appropriate medical attention for our child(ren) in the event of an accident, injury, illness. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment.

I hereby waive, release and forever discharge Mid Mo Kats from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in Mid Mo Kats activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my children is (are) physically fit and capable of participation in all activities.

For your convenience annual tuition is broken down into equal monthly installments for our FALL/SPRING session (August through end of April).

**If your child withdraws from our studio within this session period we require one month notice prior to withdrawal. If notice is not given one month in advance, the card on file will be charged for the month following student's withdrawal.

AUTOPAY
Tuition is automatically charged on the 1st of each month (August-April) to the card put on file at time of registration. A late fee of $15 is automatically charged to accounts not paid by the 10th.

To participate in our auto-billing program simply create an online profile for each dancer in your family and enter your payment information just once in our secure system. For added ease you may also enroll in optional auto-billing of the debit or credit card of your choice.

PAYMENT METHODS
Services may be paid for by cash, check, ACH, mastercard, visa and discover cards.

NOTE: All customers must have a valid credit card on file.

**A card is required to be on file, but fee may be paid via cash or check if worked out with studio director prior to class start.

I give permission for my child’s photographs/videos to be used for publicity and advertising on the Mid Mo Kats website, Mid Mo Kats Facebook page and/or Instagram and in the community.

We have closed classes (no one besides dancers may stay during class time) due to space restraints. If there is a desire to observe a class, it must be worked out with Ms. Kim prior to that class.

Do not drop off student more than 5 minutes prior to his/her class start time. Please pick up your dancer promptly. We need to utilize our space and instructors for dancers who are in class and we are not a daycare.

No cell phone use during class. If this rule is not followed, cell phones may be confiscated and returned at the conclusion of class.

Class attire should be followed according to class type. Please see class description or resource tab on parent portal or email us for information regarding proper class attire.

Please send a water bottle with your child for use during class time.

To ensure that our class time is maximized, any parent/director/instructor communication should take place outside of class time unless it is an emergency.

If your child is sick (fever, contagious, vomiting, etc.), please do not send him/her to class.

If a student misses scheduled class time a make-up lesson may be available. Please contact studio director or look in our parent portal for make-up options.

Costume information will be given out as soon as it is available for each fall, spring, and competitive class. No costumes are required for students in our summer session.

In the event of an emergency, the primary contact for each dancer will be notified. Please make sure contact information remains updated in our parent portal.
