I agree and acknowledge that I/our child have/has voluntarily applied to attend and participate in all ttgsf activities and events.

I understand that dance training for ttgsf’s students requires a sustained, repetitive, vigorous physical activity, usually performed on a hard or lightly padded surface without protective footwear (i.e. athletic shoes). It is also understood that dance instruction involves kinetic corrections that may include physically touching the student as part of regular class work and rehearsals. I understand that participants engage in a broad range of quick movements, bending, twisting, running, leaping and lifting, squating, which place extreme demands on the human body, including stress of joints and ligaments, repetitive impact, and occasional falls, slips and collisions with other participants and objects. I/we also understand that the ttgsf’s facility at City Dance Studios is not staffed to monitor and supervise the activities of its students at all times, and the tuition schedule does not contemplate constant supervision.

I also understand that there are inherent risks of serious personal injury involved in all of the above activities as well as in the general participation in ttgsf activities and events. I voluntarily assume and accept such risks of personal injury and illnesses arising from my/our child’s attendance and participation in such activities and events. I hereby release ttgsf, its trustees, employees and agents from all actions, claims or demands that I and my heirs or representatives now have or may hereafter have for personal injuries or property damage resulting from my/our child’s attendance at and/or participation in ttgsf activities and events. I agree that this release includes personal injury or property damages caused in whole or in part by negligence, active or passive, of ttgsf activities, its trustees, employees and agents. This release does not apply to liability for willful injury or fraud. This permission and release shall remain effective through the end of the academic year, unless and until a written revocation is delivered to ttgsf.

Waiver and Release of Liability
In consideration for being allowed to participate in this Activity provided by ttgsf, I release from liability and waive my right to seek damages from ttgsf, its trustees, employees and agents (collectively “ttgsf”) from any and all claims, including claims of the ttgsf’s negligence, resulting in any physical injury, illness (including death) or economic loss I may suffer or which may result from my participation in ttgsf’s Activity, travel to and from the Activity (including air travel), or any events incidental to this Activity.

This applies to all Participants 18 years and older:
I have read this document, am of legal age and I am signing it freely. I understand the legal consequences of signing this document, including (a) releasing ttgsf from all liability on my and the Participant’s behalf, (b) waiving my right to seek damages from ttgsf, (c) and assuming all risks of participating in this Activity, including travel to and from the Activity (including air travel) or any events incidental to this Activity. I understand that I am responsible for my obligations and acts. I agree to be bound by these terms.

Parent/Guardian signature required if student is under the age of 18. This applies to all Participants under 18:
I am the parent or legal guardian of the Participant. I have read this document, and I am signing it freely. I understand the legal consequences of signing this document, including (a) releasing the ttgsf from all liability on my and the Participant’s behalf, (b) waiving my and the Participants’ right to seek damages from ttgsf, (c) and assuming all risks of Participant’s participation in this Activity, including travel to and from the Activity (including air travel) or any events incidental to this Activity. I allow the Participant to participate in this Activity. I understand that I am responsible for the obligations and acts of the. I agree to be bound by these terms.
