I acknowledge that dance classes involve physical exertion and carry the risk of injury.
I certify that the dancer is physically fit to participate and have not been advised otherwise by a medical professional.
As the legal parent or guardian , I release and hold harmless RBS Dance Company its owners and operators from any and all liability, claims, demands and causes of action whatsoever arising out of or related to any loss, damage, injury, that death that may be sustained by the participant and/or the undersigned while in or upon the premises under the control and supervision of RBS Dance Company.
It is required that all dancer’s be covered by their own family insurance policies.
