Participation Waiver and Release
1. Assumption of Risk
I understand that dance involves physical activities that pose risks (e.g., falls, sprains, strains). I knowingly assume
all such risks for my child during classes, rehearsals, performances, and studio events.
2. Release & Waiver of Liability
I hereby waive and release The Dance Spot of North Jersey, including owners, instructors, staff, agents, and
affiliates, from all claims or causes of action for injuries, damages, or losses resulting from my child's participation
3. Enrollment & Payment Terms
- I understand that enrollment is not valid until the non-refundable registration fee of $45 and the first month’s
tuition are paid in full.
- Tuition is billed monthly and remains the same regardless of holidays or missed classes. I agree to set up auto-
pay, or understand that payments must be received by the 5th of each month, or a $25 late fee will be applied.
4. Medical Authorization
I confirm my child is physically fit for dance activities. If injury or illness occurs, I authorize studio staff to seek
medical treatment at my expense and agree to promptly pay any related costs.
5. Media & Photography Release
I grant permission for the studio to photograph or video my child during classes or events for use in marketing,
instruction, or social media—unless I opt out separately.
6. Studio Policies & Conduct
I (and my child) agree to abide by all studio policies and instructor guidance. I acknowledge that violations may
result in dismissal from class without refund.
7. Agreement & Electronic Consent
I understand this waiver is a legally binding contract including payment obligations.
By signing this agreement,
I have carefully read and voluntarily agree to all terms. I agree to hold The Dance Spot completely harmless,
including financial responsibility for any injuries sustained, regardless of the cause or circumstances. I will do all in
my power to obey all staff and all safety requirements, and I will seek clarification if necessary. I also agree that by
signing this waiver, I have joined The Dance Spot of North Jersey on your own free will. I was not solicited by any
staff members, owners or affiliates of The Dance Spot to be a client here.
