ÉLEVÉ DANCE ACADEMY LIABILITY WAIVER & RELEASE AGREEMENT
1. ASSUMPTION OF RISK & LIABILITY RELEASE I, the undersigned parent or legal guardian, acknowledge that participation in dance classes and related activities at Élevé Dance Academy (“the Academy”) involves inherent risks, including but not limited to, physical injury, muscle strains, sprains, fractures, or other unforeseen incidents. I voluntarily assume all risks associated with my child's participation.
I hereby release, waive, and discharge Élevé Dance Academy, its owners, instructors, staff, and affiliates from any liability, claims, demands, or causes of action arising from injury, illness, or damages sustained by my child while participating in any Academy-related activities, whether on or off the premises.
2. MEDICAL EMERGENCY CONSENT In the event of a medical emergency, I authorize Élevé Dance Academy staff to obtain medical treatment for my child if I cannot be reached. I understand that I am responsible for all medical expenses incurred as a result of any such treatment.
3. MEDIA RELEASE I grant permission for Élevé Dance Academy to take photographs and/or videos of my child during classes, rehearsals, performances, and Academy events. I understand that these images may be used for promotional materials, social media, and the Academy’s website.
4. STUDIO POLICIES AGREEMENT I have read and agree to adhere to all Élevé Dance Academy policies and procedures, including but not limited to attendance, dress code, tuition payments, and behaviour expectations. I understand that failure to comply may result in my child’s dismissal from the Academy.
5. ELECTRONIC SIGNATURE CONSENT By electronically signing this document, I confirm that I have read, understand, and agree to the terms outlined in this waiver. I acknowledge that my electronic signature carries the same legal effect as a handwritten signature.
