Waiver of Liability (required for classes and trials)
As a participant in any program or class of Tippi Toes® Dance Company and Tippi Toes Greater Milwaukee, I recognize and acknowledge the nature of the activities my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that Tippi Toes® or my child's daycare or school is not in any way responsible for making such a determination.
I agree to waive and relinquish all claims that I or my minor child may have as a result of participating in any program or class against Tippi Toes® Dance Company, Tippi Toes Greater Milwaukee, my minor child's childcare/school, and its officers, agents, servants, employees, independent contractors. I further agree to indemnify and hold harmless and defend my minor child's school or childcare facility, Tippi Toes® Dance Company and Tippi Toes Greater Milwaukee, and its officers, agents, servants, employees, and independent contractors from my claims resulting from injuries including death, damages, and losses sustained by me or my minor child that arise out of, in connection with, or in any way associated with the activities of this program.
Tuition, Fees, and Cancelation
I agree to pay registration, monthly tuition, and recital fees (if applicable) on time and agree to pay a late fee if payment is not received by the due dates. I understand the credit card convenience fees are passed on to the customer. The customer may contact Tippi Toes Greater Milwaukee if I prefer to pay a semester in advance via cash or check. I agree that tuition is non-refundable & non-transferrable, that tuition is not prorated in months that have closed days due to holidays or unforeseen closures, and that no refunds or adjustments are made due to absences. Tippi Toes encourages parents to use ACH payment method for reduced processing fees of 1% + .20 cents per transaction.
Policy for Discontinuing Enrollment: Tippi Toes classes are offered as ongoing monthly classes with monthly tuition. Written notification must be emailed to greatermilwaukee@tippitoesdance.com by the 25th day of the previous month in order to stop the auto payment for the following month. Once tuition is drafted on the 1st, it is non-refundable. If participating in a predetermined season enrollment would end at season end date.

Media Release:
I hereby permit the use of my, or my daughter/son’s IMAGE to be photographed, videotaped, or otherwise recorded for use in Tippi Toes Dance Company’s® publicity or educational materials and allow pictures to be shared with me in the class portal. These materials include but are not limited to photographs of classes and performances, newsletters, various other publications, Tippi Toes Dance Company’s® internet webpage and social media outlets, and videos of classes or performances. We respect your privacy and Tippi Toes Greater Milwaukee will NOT put the names of the dancers or the locations where they take classes in their publicity information.
