POLICIES:
• All classes are subject to cancellation, pending enrollment.
• All fees must be paid in full by enrollment date for class. There is a $15 charge for returned checks.
• No credit will be given for missed classes.
• Parents are expected to screen their own dancers for COVID 19 or other transmissible disease symptoms and not attend class if any symptoms are shown or if developed during class, we may ask the student to leave.
WAIVER OF LIABILITY
I have received, read, and will adhere to the policy statements of Minocqua Dance, LLC.
In consideration for the dance instruction and activities to be received from Dana Bostrom, dba. Minocqua Dance, LLC
I agree to the following:
1. I understand and acknowledge that the dance instruction and activities in which I am about to voluntarily engage, bear certain known risks and unknown risks, which could result in personal injury to myself or through my actions, to others.
2. I represent that I have no health or physical condition that would prevent me from engaging in the proposed dance activities or increase the risk of injury to myself, or others.
3. I understand that by signing this agreement I will have no right to make a claim or file a lawsuit against Dana Bostrom, dba Minocqua Dance, LLC or its agents, employees and representatives for injuries or damages incurred by me even if caused by the negligence of Minocqua Dance, LLC. My signature indicates that I have read this entire document, understand it completely, and voluntarily sign it.
