WAIVER & RELEASE:
I have full knowledge, and voluntarily assume, the risks involved in training for and participating in the program. I represent that my dancer is physically fit and able to participate. In consideration of signing this registration form, I, for myself and anyone entitled to action on my behalf, waive and release Elite Guam Dance Academy, their sponsors, owners, instructors, employees, representatives, officers, and successors from all claims/liabilities of any kind arising out of my dancer's participation in the program. I hereby attest and verify that I am the parent/legal guardian of my dancer, on whose behalf I make this registration, and I grant my permission for their participation in the classes.

USE OF PHOTOS/VIDEOS:
I give full permission to Elite Guam Dance Academy to use photos and videos of the participant listed on this registration form taken during classes, performances, photo shoots, gatherings and the like for marketing, advertising, studio displays, promotions and other related items for the purpose of promoting Elite Guam Dance Academy and its dance program, so long as it is done in good taste.
