1. Acknowledgment of Risk
I, the undersigned, understand that participation in dance classes, rehearsals, and performances at Infinite Dreams Dance Studios involves physical activity that carries a risk of injury. I acknowledge that I/my child is voluntarily participating in these activities and agree to assume all risks, known or unknown, associated with participation.
2. Release of Liability
I hereby release, discharge, and hold harmless Infinite Dreams Dance Studios, its owners, instructors, employees, and representatives from any and all claims, demands, damages, or causes of action arising from injuries, illnesses, or other incidents that may occur during my/my child’s participation in dance classes, performances, or any other activities associated with the studio.
3. Medical Consent
In the event of an emergency, I authorize Infinite Dreams Dance Studios to seek medical treatment for myself/my child. I understand that I am responsible for all costs associated with medical treatment.
4. Photo/Video Release
I grant Infinite Dreams Dance Studios permission to use photographs and videos of me/my child for promotional purposes, including but not limited to social media, websites, and printed materials. I understand that I will not receive compensation for the use of these materials.
5. Studio Policies
I agree to adhere to the studio’s policies, including attendance, attire, behavior, and tuition payment terms. I understand that failure to comply may result in dismissal from the program.
6. COVID-19/Health Risks
I acknowledge that participating in group activities, including dance classes, may pose a health risk, including but not limited to exposure to communicable diseases such as COVID-19. I agree to follow the studio’s health and safety guidelines to minimize risk.
