I, personally and on behalf of my child, understand and agree that participating in any dance lesson, rehearsal, performance, or workshop may result in physical injury or in some cases death. This agreement releases Adaptive Dance Education and Performance of Texas LLC from all liability relating to injuries that may occur during dance lessons, rehearsals, performances, or workshops. By signing this agreement, I, personally and on behalf of my child, agree to hold Adaptive Dance Education and Performance of Texas LLC entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.
By signing below I forfeit all right to bring a suit against Adaptive Dance Education and Performance of Texas LLC for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.
I grant to Adaptive Dance Education and Performance of Texas LLC, its representatives and employees the right to take photographs and videos of me, my child, and my property in connection with the above-identified subject. I authorize Adaptive Dance Education and Performance of Texas LLC, its assigns and transferees to copyright, use, and publish the same in print and/or electronically.
I agree that Adaptive Dance Education and Performance of Texas LLC may use such photographs and videos of me, my child, and my property with or without my name or my child’s name and for any lawful purpose, including but not limited to such purposes as publicity, illustration, advertising, and Web content.
I have read, understand, and agree to the above.

AUTOPAY ENROLLMENT & AUTHORIZATION AGREEMENT

This Autopay Enrollment & Authorization Agreement (“Agreement”) is presented by Adaptive Dance Education and Performance of Texas LLC (“ADEPT”). By selecting the checkbox and submitting this form, the Customer acknowledges and agrees to the terms outlined below, authorizing ADEPT to automatically charge the designated payment method for dance class tuition.

1. PAYMENT AUTHORIZATION

I, the Customer, authorize Adaptive Dance Education and Performance of Texas LLC (ADEPT) to automatically charge my selected payment method on the 3rd of each month for the tuition amount displayed in my Studio Pro Portal Account. I acknowledge that tuition fees may vary based on my enrollment and will be reflected accordingly in my account.

3. PAYMENT DETAILS

Amount to be Charged: Tuition amount as shown in the Customer’s Studio Pro Portal Account

Frequency of Payments: Monthly

Billing Date: Between the 1st and 7th of each month

Payment Method: Customer’s selected and editable payment method within the Studio Pro Portal

4. CANCELLATION POLICY

I understand that I may cancel my autopay enrollment by submitting a written request at least 10 days prior to the next billing date (by the 23rd of the previous month). Cancellation requests must be submitted via email to [insert ADEPT’s email] or delivered in writing to ADEPT. Cancellation will take effect at the beginning of the next full month following the approved request. I acknowledge that tuition already processed for the current month is non-refundable.

5. CUSTOMER RESPONSIBILITIES & LIABILITY DISCLAIMER

I acknowledge that I am responsible for ensuring that my selected payment method has sufficient funds available for the transaction. ADEPT is not responsible for any overdraft fees, declined transactions, or other bank-related charges. If a payment is declined, I understand that I may be subject to a late payment fee and may be required to update my payment method promptly.

6. AGREEMENT & ACCEPTANCE

By selecting the checkbox and submitting this form, I confirm that I have read, understand, and agree to the terms outlined in this Agreement. I authorize ADEPT to process recurring tuition payments as specified above.

Congratulations on your decision to participate in dance lessons! In order to maximize progress, it will be necessary for you and your child to follow your instructor’s guidelines during supervised and (if applicable) unsupervised practice. During your class, every effort will be made to assure your safety. However, as with any physical activity, there are risks, including increased heart stress and the chance of musculoskeletal injuries. In participating in this program, you agree to assume responsibility for these risks and waive any possibility for personal damage. You also agree that you will communicate with the instructor any supports needed and/or concerns you have about safety during participation in the program. With this information, instructors can take appropriate steps to avoid injury. By signing below, you accept full responsibility for your own health and well-being AND you acknowledge an understanding that no responsibility is assumed by the leaders of the program.

Terms and Conditions
1. Class Cancellations and Late Policy
If your teacher cancels a lesson we will skip over that week’s lesson and resume the following week. You will not be charged for the skipped lesson.

Refunds for missed lessons are not available when paying monthly. Lessons may be made up upon request, if a similar age/level group class is available for the make up lesson.
Students arriving late will receive the remaining scheduled session time, unless other arrangements have been previously made with the instructor.
2. Payment Policy
Payment for the following month of classes will be posted to accounts and Auto-pay will be run between the 1st and 7th of the month. Payment will be considered late after the seventh day of the month. A late fee of $15 will be added to the account after the 7th day of the month. Clients who fall two months behind on payments will not be allowed to continue lessons until the account is paid in full, including late fees.
You account may be credited and the following month’s payment adjusted if your instructor cancels classes unexpectedly.
Prorated tuition is only available during your first month enrolled. Tuition is already averaged to account for months with fewer classes due to holidays.

3. Un-enrollment and Cancellation Policy
You may end your participation in classes at any time. Please note that there will be no refunds for tuition and fees paid before cancellation. To avoid being charged for the next month of classes, you must cancel your tuition auto-payment at least 7 days before the start of the new month. Failure to do so will result in the next month’s tuition being charged as scheduled. It is the responsibility of the participant to ensure cancellation is submitted in a timely manner.

4. Attachments
You have read and agreed to the Liability Waiver and Photo Release which are available on your Parent Portal to and incorporated into this agreement as if fully set forth herein.
