North GA Clogging & Dance Co. Studio Waiver & Financial Agreement

1. Assumption of Risk
I understand that dance training, conditioning, and related activities involve physical movement that may include stretching, jumping, lifting, and other athletic actions. I acknowledge that participation carries inherent risks, including but not limited to:

Muscle strains or sprains
Falls or collisions
Injuries resulting from physical exertion
I voluntarily assume all risks associated with participation in classes, rehearsals, performances, and any other studio activities.

2. Release of Liability
I hereby release and hold harmless North GA Clogging & Dance Co., its owners, instructors, staff, contractors, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur while participating in studio activities, whether on or off studio premises.

3. Photo & Video Release (Optional)
I grant permission for North GA Clogging & Dance Co. to use photographs or video recordings of myself/my child for promotional, educational, or marketing purposes. If not, I will submit in writing (letter or email) that I do not wish for myself/ child to show in promotional, educational, or marketing purposes or mark in Dance Studio Pro to "opt out" of Public photos.

4. Studio Policies
I understand that North GA Clogging & Dance Co. is committed to maintaining an inclusive, welcoming, and supportive environment for all dancers, families, and staff. Kindness, respect, and encouragement are expected at all times. Bullying, discrimination, or unkind behavior of any kind will not be tolerated in or out of the studio including social media, etc. All participants are expected to contribute to a positive and uplifting studio community.

General Conduct
I agree to follow all studio rules, safety guidelines, and instructor directions. I understand that failure to comply may result in dismissal from class or studio activities without refund.

5. Financial Policies
Tuition & Billing:
The billing cycle begins on the 1st of every month. Tuition is due by the 15th of each month.
Late payments may result in additional fees until the account is current.

Discontinuation of Classes
I understand that a 60‑day notice is required to discontinue classes. If I discontinue with less than 60 days’ notice, I understand that I am still responsible for paying tuition to fulfill the full 60‑day period from the date notice is given. *Trial/ 6 week intro classes not included.

Temporary Breaks / Holds
If I need to pause classes for 1 (or more) months, my spot may be held for the amount equal to 1/2 tuition per month hold fee.
During a hold period, regular tuition will not be charged, but the hold fee must be paid monthly to maintain enrollment status and class placement.

Refund Policy
I understand and agree that there are no refunds for tuition under any circumstances, including but not limited to missed classes, schedule changes, withdrawal, or dissatisfaction.

General Acknowledgment
I agree to comply with all billing procedures and understand that these financial policies are in place to ensure consistent scheduling, staffing, and class availability for all students.
