1. Assumption of Risk & Release of Liability

I, the undersigned participant (or parent/guardian if under 18), understand and acknowledge that participation in dance, fitness, and other physical activities at Phoenix Dance Company involves inherent risks, including but not limited to falls, injuries, muscle strains, sprains, fractures, and other physical conditions. I voluntarily assume full responsibility for any risk of injury or harm that may result from my/my child’s participation in any Phoenix Dance Company program, class, or event.

I hereby release, waive, and discharge Phoenix Dance Company, its owners, instructors, employees, and affiliates from any and all claims, liabilities, costs, or expenses arising from injury, loss, or damage to person or property that may occur before, during, or after participation in activities at Phoenix Dance Company.

2. Medical Authorization

I authorize Phoenix Dance Company staff to obtain medical treatment for me/my child in the event of a medical emergency. I understand that every effort will be made to contact me first. I assume full financial responsibility for any medical care or treatment deemed necessary by a healthcare professional.

3. Media Release

I acknowledge that Phoenix Dance Company has the right to capture and use photographs, videos, and other media of me/my child for promotional purposes, including but not limited to social media, website content, and marketing materials. I understand that participation in Phoenix Dance Company activities constitutes my agreement to this media release, and I waive any claims for compensation related to the use of these materials.

4. Payment & Refund Policy

I acknowledge that tuition and fees for classes, workshops, and events are non-refundable and must be paid according to Phoenix Dance Company’s payment policies. Missed classes are not subject to refund, proration, or credit. Regardless of attendance, I understand that I am responsible for the full tuition amount as agreed upon at the time of enrollment. Makeup classes may be offered at the discretion of Phoenix Dance Company, subject to availability.

5. Code of Conduct

I understand that respectful behavior is expected at all times within Phoenix Dance Company. Any inappropriate, disruptive, or harmful behavior may result in dismissal from classes or programs without refund.

6. Acknowledgment & Agreement

I HAVE READ AND UNDERSTAND THIS WAIVER AND RELEASE FORM. I VOLUNTARILY AGREE TO ITS TERMS AND SIGN IT WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.

I acknowledge and agree to all terms outlined in this waiver.
