I understand that participation in dance activities involves some risk of injury. By signing below, I release Backstage Dance Studio 5678, its instructors, and staff from any and all liability for injuries, accidents, or damages that may occur while participating in studio activities.

I authorize Backstage Dance Studio 5678 to photograph or record videos of my child during classes, rehearsals, and performances. These images or videos may be used for promotional materials, social media, and the studio website.
