Powerhouse Dance Liability Waiver and Release Form
Acknowledgment and Assumption of Risk
I, the undersigned, understand that participation in dance activities at Powerhouse Dance involves physical movement and exertion, which may result in injury. I acknowledge and accept the inherent risks associated with dance, including but not limited to falls, collisions, muscle strains, sprains, and other physical injuries. I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation.
Release of Liability
I hereby release, discharge, and hold harmless Powerhouse Dance, its owners, instructors, employees, and representatives from any and all claims, liabilities, damages, or expenses arising from my participation in dance classes, rehearsals, performances, or related activities, including any claims due to negligence.
Medical Authorization
In the event of an emergency, I authorize Powerhouse Dance and its staff to seek medical treatment on my behalf if I am unable to provide consent. I understand that I am responsible for any medical expenses incurred as a result of any injury.
Photo and Video Release
I grant Powerhouse Dance permission to use photographs and videos of me taken during classes, performances, or events for promotional purposes, including social media, websites, and marketing materials. I understand that no compensation will be provided for such use.
Agreement and Signature
By signing below, I acknowledge that I have read, understood, and agree to the terms of this waiver. I affirm that I am of legal age to sign this waiver or that my parent/guardian has signed on my behalf.
