At Studio B Dance Company, we are committed to maintaining a positive, respectful, and inclusive environment for all students, families, and staff. Bullying, harassment, or exclusion of any kind—whether verbal, physical, emotional, or online—will not be tolerated. This includes actions that undermine another student’s confidence, comfort, or sense of belonging.
We reserve the right to dismiss any student from the program if they engage in bullying behavior or create a toxic or unsafe environment for others. Similarly, the behavior of parents, guardians, or family members must reflect the values of our studio. Any actions—whether directed at staff, other students, or fellow parents—that are disrespectful, hostile, or detrimental to our community may also result in removal from the studio.
Our goal is to foster a culture of kindness, support, and encouragement, where every dancer and family feels safe and valued.

1. Acknowledgment of Risk
I, the undersigned, hereby acknowledge that participation in dance, tumbling, acrobatics, physical conditioning, and other related activities at Studio B Dance Company (hereafter “Studio B”) involves inherent risks of injury. These risks include, but are not limited to, slips, falls, strains, sprains, fractures, concussions, or other physical injuries. I understand that while Studio B staff will make every reasonable effort to provide a safe environment, accidents may occur.

2. Voluntary Participation
I confirm that my child (or I, if over 18) is in good health and physically capable of participating in dance and/or tumbling classes, camps, performances, or other activities. Participation is entirely voluntary.

3. Waiver and Release of Liability
In consideration of being allowed to participate in any way in the programs, events, or activities at Studio B, I, on behalf of myself, my child(ren), my heirs, personal representatives, and assigns, hereby release, discharge, and hold harmless Studio B Dance Company, its owners, instructors, employees, contractors, agents, and affiliates from any and all liability for injury, illness, accident, or death that may occur while participating in any Studio B activity, whether on or off the premises.

4. Medical Authorization
In the event of an emergency where I, or my emergency contact cannot be reached, I hereby authorize Studio B staff to secure emergency medical treatment for my child or me, and I agree to be financially responsible for any costs incurred.

5. Special Needs and Accommodations
Studio B Dance Company is committed to fostering an inclusive environment and will make reasonable efforts to accommodate participants with special needs or disabilities in accordance with applicable laws. I understand that it is my responsibility to notify Studio B in writing of any physical, medical, cognitive, behavioral, or other needs that may require accommodation or additional support. While Studio B staff will make every effort to provide appropriate accommodations, I acknowledge that some limitations may apply due to the nature of physical activities and available resources. I agree to work collaboratively with Studio B staff to determine the most effective and safe way to support participation.

6. Severability
I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of the State of Utah. If any portion of this agreement is found to be invalid or unenforceable, the remaining sections shall continue in full force and effect.
