Policies and Student Waiver Agreement
TUITION POLICIES
Fall-Spring class tuition: IT IS AGREED THAT, AT THE DATE OF REGISTRATION AND WITH THE PAYMENT OF THE ANNUAL REGISTRATION FEE, YOU ARE COMMITTING TO 8 WEEKS OF CLASSES AND TWO MONTHS OF PAYMENTS. If, after the 8 week commitment is fulfilled, the student wishes to drop a class they are enrolled in, withdrawal in writing is needed a minimum of 2 WEEKS in advance of the next auto-billing (the first of the month). If the written withdrawal is not received at least 2 weeks in advance of billing, the card on file will be auto-charged as per the agreement listed below:
Registration for our seasonal classes (that runs September-May) opens June 1. The cost to hold your place in the classes is the registration fee ($30/student or $40/family) and is due at the time of enrollment. Total annual dance tuition is divided equally into nine payments, automatically charged to the card on file on the first of every month, September 1st - May 1st. Music lesson tuition is charged monthly on a per-lesson basis. If a family chooses to pay with cash or a check, that payment must be given prior to the first of the month. An incidental credit card must be on file with the Dance and Music Academy, and if no other payment method is received by the first of the month, that card will be auto-charged. When a student is registered, monthly payments will begin on September 1. If the card on file has been declined and payment for that month is not received by the 10th of that month, a $15 late charge will be applied to the account. You will be notified via email if your card is declined on the 1st of the month.
Summer season tuition: Tuition for the summer session is to be paid in full and due upon the registration date. TUITION IS NON-REFUNDABLE AND NOT TRANSFERABLE TO OTHER STUDENTS OR SEASONS. Paid tuition may be applied to an alternate summer session program IF the program is not full. There are no registration fees in our summer programming.
Cleaning sessions and private lessons tuition: Tuition for a cleaning session or private lesson with an instructor is due upon booking of the session. Cleaning sessions or private lessons canceled with fewer than three days notice are forfeited and will not receive an account credit or be rescheduled; cleaning sessions or private lessons canceled with more than three days notice will receive a full account credit.
MAKE-UP CLASSES
Fall-Spring Session: Missed dance classes may be made up in any other dance class of an appropriate level during the same season in which the missed class occurred; a maximum of 5 make-up dance classes is allowed. Makeups must be scheduled in advance. If a class is full, you will be added to the makeup wait list. Notifying the studio of your child's planned absence in advance is incredibly helpful so that we can open up space for students on the makeup wait list.
Summer Session: For session classes (recurring weekly class) students are eligible for one makeup class during the 5-week session.
COMMUNICATIONS
The Dance and Music Academy communicates with families via email. Please make sure the email address you provide us with is to an account you check regularly. Please add trifectadma@gmail.com to your safe sender list. If you do not receive a welcome email upon enrollment, please check your spam folder.
DAMAGE/INJURY RELEASE
Release: “I am aware that by registering in this program, or by registering my minor child/ward, I will be waiving my rights and/or the rights of my minor child to all claims for injuries I or my minor child/ward might sustain while participating in this program and I will be required to indemnify, hold harmless, and defend Dance and Music Academy, Inc for any claims arising out of participation in said program.”
Risk of Injury: “As a participant in the program, or as a parent/legal guardian of a participant under 18 years of age, I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume the full risk of injuries, including death, damage or loss which I may sustain as a result of participating in any and all activities associated with this program.”
Waiver of injury Claims: “I agree to waive and relinquish any and all claims I may have arising out of, connected with, or in any way associated with the activities of the program.
Release from Liability: “I do hereby fully release and discharge Dance and Music Academy, Inc and its officers, agents, and employees from any and all claims from injuries including death, damage or loss which I or my minor child/ward may have or which may occur on account of participation in the program.”
Indemnity and Defense: “I further agree to indemnify, hold harmless and defend Dance and Music Academy, Inc and its officers, agents, and employees from any and all claims from injuries including death, damages and losses sustained by me or my minor child/ward and arising on the of, connected with, or in any way associated with the activities of this program.”
In the event of any emergency: “I authorize Dance and Music Academy, Inc to secure from any licensed hospital, physician, and/or medical professional any treatment deemed reasonable and necessary for my minor child/ward’s immediate care and agree that I will be responsible for payment of any and all medical services rendered.”
POLICY FOR STUDENTS OUTSIDE OF THE CLASSROOM
Premiere Division dancers are expected to have an adult caregiver escort them into and out of the building. The whereabouts of all other dancers outside of the classroom either before, after, or in between classes is at the sole discretion of their adult caregiver. DMA staff does not have the capacity to monitor the whereabouts of students while teaching classes and conducting regular business.
NUT-FREE POLICY
DMA is a nut-free studio. Please refrain from bringing personal food or birthday party treats containing any nuts, including any beverages containing nut milks, into the studio or at any off-site DMA events.
PHOTO/VIDEO RELEASE
“I understand that, from time to time, Dance and Music Academy, Inc., may take photographs or videos of students or performers during lessons, practices or performances. I hereby grant to Dance and Music Academy Inc a non-exclusive, royalty-free, revocable right to use still photographs or videos for testimonial and promotional purposes. I hereby release and discharge Dance and Music Academy, its employees, agents, licensees, successors and assigns from any and all claims, demands or cause of actions that it may have or may later have for libel, defamation, invasion of privacy or right of publicity, infringement of copyright or trademark, or violation of any other right arising out of or relating to any utilization of the rights granted herein.“
