Conditions of Enrolment (ALL STUDENTS)
Fees are invoiced once a term in advance (no refunds for missed classes unless doctors’ certificate is provided.) If an invoice is not issued, fees can still be paid according to the fee structure.
All payment of fees must be strictly adhered to. If accounts are in arrears at the end of each Term, classes for the following Term may only resume once payment for the previous Term has been received.
We require two weeks of notice in writing for any child who decides to leave the studio. Your account will be charged up until the end of the term, all fees are due and payable.
All choreography remains the property of Dance HQ.
No parent is to stay with a student while a class is being conducted. We have a viewing week once at the end of each term.
No child should be left without the parent being satisfied that their child is in class and under the care of the teacher nor should any child be encouraged to leave class without supervision.
Students should be picked up on time at the end of their classes. When students leave the premises, they are out of Dance HQ care. No students should wait outside the premises to be collected. All parents/guardians must come inside the building to collect.
It is important to advise of any changes to your enrolment or contact details, throughout the year.
The Declaration signed below is also the person responsible for payment of ALL fees and agrees and will abide by Dance HQ Fee Policy for 2025.
Dance HQ has a very high NO Zero Tolerance bullying Policy. which includes social media. verbal or physical. Dance HQ also has
in place a zero tolerance to publicly discrediting Dance HQ or any teachers. students. parents and office workers on any publ ic
forum. Instant dismissal from Dance HQ and legal action will commence. Please contact Kerene if you have any concerns re your
child's wellbeing during their time at Dance HQ
Dance HQ will take photographs/video footage for social media, advertising and learning purposes. If for any reason you do not want your

All dance classes can involve the risk of personal injury. Dance HQ takes all reasonable care in the conduct of its classes, so therefore does not
accept responsibility for any injury or loss of property caused whilst participants are on studio grounds.
I understand at times my child will have some physical contact with teachers for correction and other
students for support, and to form sequences throughout the dance. I give permission for this.

at Dance HQ you must take care of your own personal safety at all times.
ACKNOWLEDGEMENT OF RISKS, INJURY & OBLIGATIONS
I acknowledge that the activity I am to undertake can be a dangerous activity and that by participating in it I am exposed to certain risks.
I acknowledge and understand that whilst participating in such activity:
a) I may be injured, physically or mentally, or may die
b) My personal property may be lost, stolen or damaged.
c) Other persons participating in such activity may cause me injury or may damage my property.
d) I may cause injury to other persons or damage their property.
e) The conditions in which the activity is conducted may vary without warning.
f) I may be injured, die, or suffer damage to my property because of breaching this contract.
g) I assume the risk of and responsibility for any injury, death or property damage resulting from my participation in the activity.
RELEASE AND INDEMNITY
I participate in the activity at my sole risk and responsibility. I release, indemnify, and hold harmless Dance HQ, its teachers, and staff, from and against all and any actions or claims which may be made by me or on my behalf or by other parties for or in respect of or arising out of any injury, loss, damage or death caused to me or my property whether by negligence, breach of contract or in any way whatsoever.
Declaration:
I am a Parent/Guardian of the student named on the front of this form and I understand and agree to the above conditions and take full responsibility to ensure that all fees and medical conditions are adhered to, and ALL fees are paid on time.
