Medical Emergency Policy: The undersigned gives permission to Nina's School of Dance, Inc., its owners, operators and instructors to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I agree to provide any physical/mental problems, restrictions, or condition in the comments section below. By not placing any comments in the comments section, I declare the participant to be in good physical and mental health.

I give permission for Nina's School of Dance, Inc. to take photos of me or my child while participating in studio activities for promotional purposes. I understand that for the safety of dancers and their families, all photos and/or videos of studio classes, rehearsals and performances, including dances in studio costumes, will not be used, published or posted publicly in printed or electronic format without the express written permission of the studio and the director.

Release of Liability Policy: As the legal parent or guardian, I release and hold harmless Nina's School of Dance, Inc., DBA Nina's School of Dance, its owners, operators, instructors, and guest instructors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Nina's School of Dance, Inc., its owners, operators, and instructors or in route to or from any of said premises.
