Injury: Due to the physically active nature of dance lessons we need expressed permission for you/your child to participate in our dance classes. Waiver Of Liability: I hereby give permission for son/daughter/self to participate in Nicolle DelliColli, DelliColli Dance Center, 875 Lafayette LLC and her subcontractors’ classes and will not hold responsible either the directors, teachers, or teaching assistants for any accidents or injury which may occur in relation to the lessons. I've read the above and agree. Photo Release: I hereby allow Nicolle DelliColli, 875 Lafayette LLC and her subcontractors’ to use my students photo/video on the studios social media and website. I've read the above and agree. Dress Code: No street shoes, meaning only shoes with clean soles worn inside. Behavior: All students must be polite and respectful to each other and their teachers. If a student is disrespectful, the teacher has the right to ask the student to leave the classroom. No cell phones are to be used in class. All phones must be on silent or turned off.
