All students and their parents are aware of possible physical injury that may occur during dance classes, performances, and/or rehearsals and are willing to assume those risks. It is agreed that students and their parents/legal guardians will not hold Formatons Haus of Dance, its directors, teachers, or employees liable for injuries sustained while in attendance or while participating in any Formatons Haus of Dance activity. We agree to abide by all rules and regulations set forth for any specialty group we register our child/ren for. We give permission to Formatons Haus of Dance, to use pictures or videos of our child for advertising purposes. I understand that I am responsible for all late fees incurred when any payments are not made on time. I understand that if I do not pay on time, my child will not be allowed to perform.
