At Revolution Dance Academy, our goal is to provide a positive and supportive environment for all of our staff, parents, and dancers. Please review the following guidelines in our Code of Conduct and Sign.

Building and Classroom Conduct:

Out of respect for our teachers and students, parents are not permitted inside the classrooms without permission from an instructor. Please do not enter a classroom to correct your child, spot your child, or “assist” the teacher in instructing your child.

All children must be supervised at all times by a parent. Siblings of any age may not be left at the studio without direct parental supervision. If this occurs, you will be called and asked to come and pick up your unattended child.

For everyone’s safety and comfort, please do not allow your children to run, shout, or roughhouse in the building.

Please keep our studio clean! Eating is only permitted in designated areas. Please throw away any trash or food items in the appropriate receptacles.

Please dispose of any bathroom waste (including sanitary napkins) appropriately.

Studio Conduct (applies to all current Revolution Dance Academy families, whether inside or outside of the studio.)

Please understand that violation of these guidelines is cause for immediate dismissal from our studio, at the discretion of the studio owners.

Conflict Resolution:
Any conflicts or concerns involving staff members, parents, or dancers should be discussed only with the appropriate staff members during a scheduled appointment time. You may schedule an appointment at the front desk to meet with studio directors or teachers. Discussing grievances in common areas with other parents does not exemplify the positive environment we wish to provide at our studio, and will not be tolerated. Doing this not only sets a negative example for our dancers, it reflects poorly upon those involved in the discussion, as well our studio.

Social Media:
Posting defamatory or negative remarks about Revolution Dance Academy, its staff members, or other Revolution Dance Academy families on any social media platform will not be tolerated and is cause for immediate dismissal from the studio.

Dispute Resolution:
We value each and every dancer and parent, and wish to resolve any issues in the most amicable manner possible. Please understand that approaching another parent or student with the intent to cause conflict or behave in a disrespectful manner, disrupts our ability to create a positive and nurturing learning environment for our students. This type of behavior will be cause for immediate dismissal from the studio, at the discretion of the studio owners. Any disputes should be handled by first scheduling a meeting with appropriate staff members.

Code of Conduct Enforcement Policies:

When a parent or dancer violates this code of conduct, or any other stated policies of Revolution Dance Academy, the following steps will be taken:

A warning will be issued wherein the policy violation will be discussed with the parent or dancer, by appropriate staff. Together, we will discuss how to avoid future violations of studio policy. Parents and dancers will sign an agreement indicating that they understand the policy violation and what is expected of them moving forward.

At the discretion of studio owners - further violations of policies set forth in this Code of Conduct, or any other stated policies of Revolution Dance Academy, will be cause for immediate dismissal from the studio.

I have read and agree to follow Revolution Dance Academy’s Code of Conduct policies. I understand that failure to adhere to the Code of Conduct, or any other Revolution Dance Academy policy, is cause for immediate dismissal from Revolution Dance Academy.

Parent Release Form for Photography and Videography

I, the undersigned, give permission for Revolution Dance Academy to use video footage and / or photographs of my child/ward, _________________________________. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to:

- printed materials (eg - brochures and newsletters)
- online and offline advertising and promotion
- videos and digital images such for use on Social Media.

By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Revolution Dance Academy for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.

Health Information and Medical Release/Waiver Form

I, _________________, the parent/guardian of _________________________, acknowledge that participation in dance is potentially dangerous and the is an inherent risk of injury involved.

In allowing my child to participate in Revolution Dance Academy activities, I hereby assume all the risks associated with the performing arts. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release Revolution Dance Academy and it’s employees of any and all liability which may arise as a result of my child’s participation in activities at Revolution Dance Academy.
