Liability Waiver Form:

I realize that participation in dance classes and activities could involve some possible personal injury. By signing this release form,
I (the dancer’s parent/guardian) assume all risks related to the use of any and all spaces used by A Time To Dance, Dance Studio,
I/WE, the parents/Legal guardian of the registered student agree not to hold Victoria Scherillo, A Time To Dance,Dance Studio, Officers, landlords, dance instructors, assistants and employees responsible for any injuries, loss, accident or damages that might occur while my child/children are attending dance lessons or performances. I further state that my child/children is/are in good health and are not restricted from physical activities, such as dance. I have read this release and intending to be legally bound have set my signature hereunder:

Tuition

REGISTRATION AND TUITION FEES:

Registration fees are a one-time payment due annually per dancer. Registration fees are $20 for the first child, $30 for family and must be paid at time of sign up. In order to enroll in a desired class and save your spot, you must complete the online registration form and pay the registration fee. Registration fees are non refundable.

All Registration fees and Tuition fees are paid at the time of enrollment via: Zelle @(toriblessedby3@gmail.com), check, credit card or ACH. Checks made payable to: A Time To Dance, Dance Studio.

** Tuition is based on a 9 month installment plan (our dance year consists of 33 weeks of paid classes). Classes will be running from September 2025 to May 2026. Tuition is non refundable and must be paid every month. We do not prorate for missed months or classes. Make up classes are available for all classes missed.
All tuition payments will be paid in 3 trimester payments,* 1st payment due at time of registration AND/OR by September 1st , 2nd payment due on December 1st, 3rd payment due on March 1st
(last payment).
If you pay by month, all tuition payments must be paid by the 1st of each month with a 10 day grace period. If tuition is not paid by the 10th of the month, a $10 late payment fee will be charged to your account. I understand that my tuition payments must be kept up to date and agree to fulfill my obligation. Please sign your name below.
