Registration & Payment Terms
By completing registration at Unity Dance Co., you acknowledge and agree that:


All class selections and totals will be reflected in your Parent Portal upon finalizing registration.
A non-refundable registration fee is due at the time of enrollment.
Tuition is fully refundable only if written notice of withdrawal is submitted on or before September 15, 2025.
After September 15, all tuition becomes non-refundable, regardless of attendance or circumstance.
We offer one optional payment plan if needed: 50% due on the first day of class, and the remaining 50% due by October 1. Late or missing payments may result in suspension or dismissal from classes. A $50 registration fee will charged to your parent account

Release of Liability & Assumption of Risk
I, the undersigned, understand and acknowledge that participation in dance classes, rehearsals, performances, camps, intensives, and all other activities associated with Unity Dance Co. (“the Studio”) involves inherent physical risks, including but not limited to falls, sprains, strains, broken bones, and other injuries which may result from participation.

I voluntarily assume all risks, known and unknown, associated with my own and/or my child’s participation. I hereby release, waive, and hold harmless Unity Dance Co., its owners, directors, employees, contractors, agents, and volunteers from any and all liability, claims, demands, or causes of action for injuries, damages, or losses—physical, emotional, or financial—sustained as a result of participation, whether caused by negligence or otherwise.

Medical Authorization & Emergency Care
I acknowledge that Unity Dance Co. staff are not medical professionals and may only administer basic first aid (e.g., ice packs, bandages) for minor injuries. No medications, including over-the-counter drugs, will be given. In the event of a medical emergency, I authorize Unity Dance Co. to call 911 and/or seek medical treatment at the nearest medical facility. I assume all responsibility for resulting medical costs.

Behavioral Expectations & Dismissal Policy
I agree to ensure that I and/or my child will follow all studio rules and conduct expectations set by Unity Dance Co. Disrespectful, unsafe, disruptive, or threatening behavior toward staff, students, or property may result in immediate dismissal without refund.

Photography & Videography Consent
I grant permission for Unity Dance Co. to photograph and/or video record me and/or my child during classes, performances, and events. I understand these materials may be used for studio marketing purposes including—but not limited to—social media, the studio’s website, advertising, and promotional content. I waive any rights to inspect or approve the final product and any right to compensation.

Personal Property Disclaimer
Unity Dance Co. is not responsible for lost, stolen, or damaged belongings of any kind, including but not limited to dance bags, water bottles, phones, shoes, and clothing. Students and families are solely responsible for securing personal items. Items left behind may be placed in a lost-and-found area temporarily, but the studio assumes no responsibility for holding, replacing, or recovering such items.

Drop-in Classes & Class Pass Policy (if applicable)
Class passes are non-refundable and non-transferable. If a class is canceled with less than 48 hours’ notice, the class credit will still be deducted.

Agreement & Signature
By signing below, I acknowledge that I have read, understood, and agree to all terms outlined in this Liability Waiver, Policy, and Consent Agreement. I understand that this agreement is legally binding and intended to protect Unity Dance Co. and its affiliates to the fullest extent permitted by law.
