By registering, I, as the parent/guardian of the Participant, agree to release Charisma School of Dance, its staff, and volunteers from any liability related to accidents, injury, or property loss during classes, events, or activities. I acknowledge that dance involves physical activity and inherent risks, and I accept full responsibility for any necessary medical treatment. I also give permission for emergency medical care if needed.
I understand that physical contact may occur between instructors and students for safety and proper instruction, and I consent to this. I also acknowledge the risk of exposure to illnesses, including COVID-19, and assume full responsibility for any associated risks.
Charisma School of Dance is not responsible for the Participant outside of scheduled class times. A $30 late pickup fee will apply if I am 15 minutes or more late to pick up my child after class.
I agree to follow the studio’s dress code and policies, and to ensure that the Participant, as well as anyone accompanying me, is aware of them. I also consent to the use of the Participant’s photos and videos for promotional purposes.
I acknowledge the financial policies, including the $25 cancellation fee for dropping a class, camp, or lab within 14 days of the start date, a $25 late payment fee for overdue tuition, and the requirement to submit a withdrawal form at least 2 weeks in advance to avoid a full month’s tuition charge.
