I am a student or parent of a student at The Studio.

I have read the studio policies on the website and agree to adhere to the terms and conditions listed.

I acknowledge that participation in dance classes involves a level of risk of personal injury and I assume all risks that may possibly occur on the premises before, during, or after classes.

I certify that my child is in good health and without physical limitations that may increase risk of personal injury.

I agree that I will not hold The Studio or any staff member liable for any personal injury or any personal property damage that may occur on the premises before, during, or after classes. I will not hold The Studio or any staff member liable for any illnesses contracted on the premises.

I agree that in the event of an extreme medical emergency, The Studio has permission to make necessary arrangements to transport my child to a medical facility if necessary to do so.

I acknowledge that my child's photograph and/or video may be used for educational and promotional marketing for The Studio, and that the accounts on which they may be used are under Public settings and may be viewed by anyone on such platforms.

I acknowledge that if I do not want my child to be featured on any public social media accounts, I will notify The Studio by selecting "Photo Opt-Out" on the Waivers & Policies page of the online portal.
