1. I, the parent, understand that physical exercise and dancing are a part of excelling in dance. These activities can sometimes involve the risk of serious injury, disability, death, and property damage. I acknowledge that these risks may result from or be compounded by the actions, omissions, or negligence of my dance instructor(s). I understand that although the dance instructor(s) will endeavor to reduce the risk of injury from dancing, the dance instructor(s) cannot guarantee that you or your dancer will not be injured. NOT WITHSTANDING THESE RISKS, I ACKNOWLEDGE THAT I AM VOLUNTARILY PARTICIPATING IN DANCING. I AGREE TO ASSUME ALL RISKS OF ILLNESS, PERSONAL INJURY, PSYCHOLOGICAL INJURY, PAIN, SUFFERING, DISABILITY, DEATH, PROPERTY DAMAGE, AND FINANCIAL LOSS ARISING THEREFROM, WHETHER CAUSED BY THE ORDINARY NEGLIGENCE OF THE DANCE INSTRUCTOR(S) OR OTHERWISE.

2. I waive and release all claims, now known or hereafter known, against the dance instructor(s) on account of person or psychological injury, illness, pain, suffering, disability, death, property damage, or financial loss arising out of or attributable to dance, whether arising out of the ordinary negligence of the dance instructor(s) or otherwise. I agree not to make or bring any such claim against the dance instructor(s) and forever release and discharge the dance instructor(s) from liability under such claims.

3. I confirm that I am in good health and proper physical condition and do not have any medical or other conditions that would impair my ability to participate in any dance routines, and I will also follow all instructions, recommendations, and cautions of the dance instructor(s) at all times during the dance season. If at any time I believe that I am no longer in proper physical condition to participate in dancing, I will immediately inform my dance instructor and possibly discontinue further dancing.

4. I hereby consent to receive medical treatment deemed necessary if I am injured or require medical attention during dancing. I understand and agree that I (the dancer) am solely responsible for all costs related to such medical treatment and any related medical transportation and evacuation. I release, forever discharge, and hold harmless the dance instructor(s) from any claim based on such treatment or other medical services.

5. This release may not be orally modified and constitutes the entire agreement of the dance instructor(s) and me concerning the subject matter contained herein and supersedes all prior and contemporaneous agreements, both written and oral, concerning such subject matter. If any term or provision of this release is deemed invalid, illegal, or unenforceable, all other terms or provisions shall remain in full force and effect.

6. BY CLICKING THE ACCEPT BUTTON, YOU ACKNOWLEDGE THAT YOU HAVE READ AND UNDERSTOOD ALL OF THE TERMS OF THE RELEASE AND THAT YOU ARE VOLUNTARILY GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE THE DANCE INSTRUCTOR(S) AND NATIA O LE PASEFIKA.

* I hereby grant permission to use my child(s) photographs for web-based, print, or social media platforms.

* I affirm that such release to the releasee does not constitute any form of compensation, including royalties arising from the photographs to my benefit.

* I understand and agree that photographs in the possession of the releasee shall become the property of the releasee. The use and publication of the photographs, however, shall conform to my rights as the subject of said photographs.

* I hereby waive my right to inspect of approve those photographs by which my likeness appears.

* I hereby hold harmless, release and forever discharge the from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by the season of this authorization.
