Uplift Dance Studio – Release of Liability Waiver
We care deeply about the safety and well-being of your dancer. Please read and acknowledge the waiver below:

Participant Acknowledgment & Consent
By signing below, I acknowledge and agree to the following:

I understand that participation in dance classes, rehearsals, and performances involves physical activity and may carry a risk of injury or property damage.

I give permission for my child to participate in all activities offered by Uplift Dance Studio, including but not limited to classes, performances, and related events.

I release and hold harmless Uplift Dance Studio, LLC, its owner (Samantha Voss), instructors, staff, volunteers, and any affiliates from all claims, liabilities, or damages that may occur as a result of participation, even if caused by negligence.

I accept full responsibility for any medical costs, injuries, or damages that may occur to my child or their belongings.

I understand this waiver applies during any activity, on or off Uplift premises (e.g., at 926 G Street, Geneva, NE, or other locations for performances).

I understand this release is intended to be as broad and inclusive as permitted by the laws of the State of Nebraska. If any portion is found invalid, the remainder shall continue in full legal force.

Emergency Authorization
In case of injury or emergency, I authorize Uplift Dance Studio staff to seek appropriate medical care for my child and to act in my absence to secure necessary treatment.

Photo & Media Release

I hereby grant permission to Uplift Dance Studio LLC to photograph and/or video record my child during classes, rehearsals, performances, special events, and studio activities.

I understand that these photographs and videos may be used by Uplift Dance Studio for promotional, educational, and marketing purposes, including but not limited to:

Social media platforms
Studio website
Printed marketing materials
Newsletters and email communications
Recital programs and event materials
Advertising and promotional campaigns

I understand that no compensation will be provided for the use of such photographs or videos.

I release and hold harmless Uplift Dance Studio LLC, its owners, employees, and representatives from any claims arising from the use of photographs or videos taken in connection with studio activities.

By selecting "I Agree," I acknowledge that I have read and understand this Photo & Media Release and grant permission for my child to be photographed and/or recorded as described above.
