1. Acknowledgment of Risk
By signing this waiver, I, the undersigned participant or legal guardian (if the participant is a minor), acknowledge that participation in classes, workshops, and events at Fuzion School of the Arts (the “School”) may involve certain risks of injury, including but not limited to physical injury, emotional distress, or personal property loss. I voluntarily assume all risks associated with participation in these activities.
2. Health and Medical Conditions
I represent and warrant that the participant is in good health and physically able to participate in the classes, workshops, and events offered by the School. I understand that it is my responsibility to inform the School of any medical conditions, limitations, or concerns that may affect the participant’s ability to participate safely.
3. Release of Liability
In consideration for being permitted to participate in the classes, workshops, and events at Fuzion School of the Arts, I hereby waive, release, and hold harmless Fuzion School of the Arts, its directors, staff, employees, agents, and volunteers from any and all claims, demands, actions, and causes of action, including but not limited to claims for personal injury, property damage, or other damages resulting from or arising out of participation in these activities, whether caused by negligence or otherwise.
4. Code of Conduct
I agree to abide by the Fuzion School of the Arts' rules and policies, including the code of conduct set forth for students and participants. This includes respectful behavior toward instructors, staff, and other participants. The School reserves the right to remove any participant from classes or events for inappropriate behavior.
