Zoetic Movement, ZM Light Competition Team Informational Packet, Rules and Regulations, and Agreement
As the parent or guardian of my dancer, I acknowledge I have received the Zoetic Movement, ZM Light Competition Team Informational Packet on the date listed below. I have reviewed and agree to the terms, and understand the rules and regulations for ZOETIC MOVEMENT Dance Company 2024-2025 competition year. This is a commitment that starts August 1st 2024 and ends July 30th 2025. I understand that I am responsible for all items indicated in the packet above and all associated fees from Dancer Fees work sheet.
Please Initial that you read and understand the following:
 _____I understand that competitive dancing can be strenuous and rigorous and thus do not hold Zoetic Movement (ZM) or any of the staff associated with ZM liable for any injury sustained while training, practicing, or performing.
 _____I acknowledge and consent, that, during the course of training and performing, video and audio media may be collected and used in a usual course for business marketing and promotions.
 _____I have read, understand and will comply with the Expected Behavior section of this packet.
 _____I understand I will keep a credit card on file. I also understand the credit card on file will be charge in the events lined out in this packet.
 _____I have read and understand the fulfillment of technical classes in relation to my dancers competition routines.
 _____I have read and understand all fees and charges associated with my child engaging in competition dance with ZM.
 _____I acknowledge that competition dates are tentative at this time and a finalize competition schedule will be rendered ASAP by ZM.
 _____I acknowledge that fees presented today are approximate and will be finalized before August 1, 2025.
 _____I commit to my dancer, team-mates and teachers, to have them at their scheduled classes and rehearsals on time.
 _____I understand that Make-up and rhinestones are a part of competitive dance.
 _____I acknowledge that fundraising is available to off-set the cost of competitive dance.
 _____I will do my best to Represent ZM while at competitions.
