Please make sure this account is set up under the PARENT'S NAME, unless the student is over 18 years old and will be responsible for their own bill. Please add EACH STUDENT in your family under the account of ONE PARENT, so all family members will show on the same account.

IMPORTANT: You MUST OPT IN for TEXT MESSAGES from the studio. This is our main line of communication to the parents with important information throughout the year.

I/My student or students choose to participate in classes, performances, workshops, and other activities at Virginia Dell School of Dance of their own free will and certify that I/they are in proper physical condition to take part in such activities.

If there are any questions about whether an activity is suitable for me/my student or students to pursue, a personal health care provider should be consulted in making that decision. If there are any known physical vulnerabilities, conditions, or injuries, I agree to discuss them with the director before participation.

By signing this document, I release Virginia Dell School of Dance and their directors, owner, students, staff, employees, volunteers, and associates from any liability or claim that I or my representatives may have against the school with respect to any bodily injury, personal injury, illness, or property loss or damage that may result from my participation at Virginia Dell School of Dance.

I voluntarily release and forever discharge and hold harmless the school from any and all claims or demands for damages, loss of services, costs and expenses, injuries, attorney fees, and any other call for reparation from any and all injury arising in any way from my participation in dance classes, performances, the use of equipment or facilities, and any activities associated with the school.

By choosing this, your child/children MAY be included in any photos or videos that are posted to the public social media accounts.

Students WILL be included in class videos that are sent out to parents and students as a way of practicing their dances at home, regardless of the status of this waiver. These will be posted to the private studio Google Drive account that only members of the class will be able to access.
