1 subject per week - 4 classes per month = $96 a month/ $24 per class
2 subjects per week - 8 classes per month = $170 a month/ $21.25 per class
3 subjects per week - 12 classes per month = $234 a month/ $19.50per class
4 subjects per week - 16 classes per month = $295 a month/ $18.43 per class

Tuition Policy
Our monthly tuition is calculated based on the number of classes offered each month. If we are closed for holidays or scheduled breaks, tuition is adjusted accordingly to reflect only the days we are open. In months with five weeks, tuition will reflect the additional classes offered during that period.

WAIVER AND RELEASE OF LIABILITY

EZPZ Kids LLC

Mommy and Me and Youth Enrichment Activities

Location: 8619 S Dixie Hwy Pinecrest, Florida

Acknowledgment and Assumption of Risk:

I, the undersigned, hereby acknowledge and agree that participation in the activities offered by EZPZ Kids LLC, including but not limited to kids fitness, science, cooking, cheer, acrobatics, art, dance, STEM, and sensory activities, involves inherent risks, including, but not limited to, the risk of personal injury, property damage, or death. These risks include, but are not limited to, physical injury resulting from strenuous physical activity, equipment use, and other participants.

I voluntarily assume all such risks on behalf of myself and my child.

Release and Waiver:

In consideration of being allowed to participate in the activities offered by EZPZ Kids LLC, I hereby release, waive, discharge, and covenant not to sue EZPZ Kids LLC, its owners, directors, officers, employees, agents, and volunteers (collectively, "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child, or to any property belonging to me, while participating in such activities or while on the premises where the activities are conducted.

Minors:

I understand that as the parent or legal guardian of the minor child listed above, it is my responsibility to ensure my child’s safety and to be aware of the risks involved in participating in the activities. I acknowledge that minors are not always able to recognize the risks involved in activities and may require additional supervision. I agree to provide such supervision or ensure that my child is adequately supervised at all times.

Indemnification:

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including court costs and attorney fees, that may incur due to my or my child's participation in said activities, whether caused by the negligence of the Released Parties or otherwise.

Medical Treatment:

In the event of an emergency, I authorize EZPZ Kids LLC to seek medical treatment for my child and agree to accept financial responsibility for any and all medical expenses incurred.

Photo/Video Release:

I grant permission to EZPZ Kids LLC to use photographs and/or video recordings of me and my child taken during activities for promotional purposes, including but not limited to social media, websites, and other promotional materials.

Membership Policies:

1. Membership Payments

• All memberships are set up on automatic payment (AutoPay).
• Payments will be processed on the 1st of every month using the payment method on file.
• If you wish to discontinue your membership, you must notify us in writing by the 24th of the month. Failure to do so will result in automatic billing on the 1st for the following month, and no refunds will be issued.

2. Make-Up Classes

• Members are allowed to make up missed classes by attending any of our other scheduled classes.
• All make-up classes must be completed within the same month as the missed class.
• Once a membership becomes inactive, members are no longer eligible for make-up classes.

3. No Credits or Rollovers

• We do not offer credits, refunds, or rollovers for missed classes.
• Unused classes do not carry over into the next month.

By enrolling in an EzPz Kids Club membership, you agree to the terms outlined in this policy.

Governing Law:

This Waiver and Release of Liability shall be governed by and construed in accordance with the laws of the State of Florida.

Signature:

I have read this Waiver and Release of Liability, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
