You must be 18 or older to register for an account.

I understand and agree that participating in any dance lesson or class, workshop, rehearsal or performance, there is a possibility of physical injury. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me (or my child) during any of Center Stage Dance Studio classes, lessons, rehearsals, performances, or activities.

I also exempt, release, and indemnify Center Stage Dance Studio, its owners, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Center Stage Dance Studio.

I understand that I should be aware of my physical limitations and agree not to exceed them.

If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.

Center Stage Dance Studio staff may occasionally take photos and/or videos of your child during class or performances throughout the year. By signing this waiver you are agreeing to allow us to post these pictures or videos to our social media outlets including instagram, facebook, tiktok, and our website.

Tuition Policy - By accepting this policy, you are agreeing to and stating that you understand the terms below.

Due Date:
Tuition will be posted each month on the 1st in your Dance Studio Pro Account. Tuition is due by the 14th of each month.

Late Fees:
Tuition payments made after the 14th of each month will have a $10 late fee added on to the tuition amount due.

Payments not made 1 week after the initial late fee has been applied, will have an additional $5 late fee applied to your account.

If tuition becomes 2 months late (the 14th of the second month), your child will not be able to attend any further classes until tuition has been paid.
