Acknowledgment and Assumption of Risk:

I, the undersigned parent or legal guardian of the minor named below, acknowledge that participation in dance classes and related activities at In The Moment Dance Studio involves inherent risks, including but not limited to risks of injury, both minor and serious, and even death. I understand that these risks may include, but are not limited to, muscle strains, joint injuries, falls, bumps and accidents during dance routines and exercises.
I willingly assume all risks associated with the minor's participation in these activities, including those caused by the negligence of In The Moment Dance Studio, its instructors, employees, or volunteers.

Release and Waiver of Liability:

In consideration of allowing the minor to participate in dance classes and related activities at In The Moment Dance Studio, I hereby agree to release, waive, discharge, and hold harmless In The Moment Dance Studio, its instructors, employees, volunteers, and any affiliated individuals or entities from any and all liability for any claims, demands, or causes of action that I, the minor, or our heirs, assigns, or legal representatives may have for any injury, loss, or damage sustained by the minor or their property arising out of or in connection with participation in these activities, whether caused by the negligence of In The Moment Dance Studio or otherwise.

Medical Consent:

In the event of an emergency involving the minor, I consent to the administration of first aid and emergency medical treatment deemed necessary by the staff or emergency personnel.

I have read this Release of Liability Waiver carefully, understand its contents, and voluntarily sign it. I am aware that by signing this waiver, I am giving up certain legal rights and remedies.

I, the parent or legal guardian of the minor named, grant permission to "In The Moment Dance Studio" to use photographs, videos, or other media in which the minor may appear. This consent includes but is not limited to the use of these media for training, as well as promotional purposes including marketing materials, social media, websites, and other forms of communication related to In The Moment Dance Studio.

I understand that these materials will be used to promote the studio’s activities and may be distributed to the public. I also understand that I will not receive any compensation for the use of such photographs, videos, or media.

Rights Granted:

Scope of Use: The photographs, videos, or other media may be used in any manner deemed appropriate by "In The Moment Dance Studio", including but not limited to advertising, social media posts, and website content.

Duration: This release is effective for the duration of the minor’s participation in activities at In The Moment Dance Studio and beyond, as long as the media is used in connection with the studio's promotional purposes.

Revocation: If I wish to withdraw this consent at any time, I must provide written notice to In The Moment Dance Studio. The request must be made in advance to ensure that the minor is not included in future photographs or media. However, I understand that any media already published prior to the receipt of such notice will not be removed or altered.

Release and Waiver:
I hereby release, discharge, and agree to hold harmless In The Moment Dance Studio, its employees, representatives, and agents from any and all claims, demands, or causes of action that I or the minor may have arising from the use of the minor's image, including but not limited to any claims for defamation, invasion of privacy, or any other cause of action.

Understanding:
I have read this Photo Release Form carefully, understand its contents, and voluntarily signed it as the parent or legal guardian of the minor. I acknowledge that I am granting permission for the use of the minor’s image in accordance with the terms outlined above.
