In exchange for participation in dance classes, online classes, private lessons, workshops, and/or dance-outs organized by O’Grady Quinlan Academy of Irish Dance, Inc. and taught by ADCRG, TCRG, Associate TC’s, and/or any other approved instructor/assistant (fitness/yoga, etc.), I agree for myself and (if applicable) for the members of my family, to the following:

1. I agree to observe and obey all posted rules and warnings and further agree to follow any oral or written instruction or directions given by any teacher, employee, representative or agent for O’Grady Quinlan Academy of Irish Dance, Inc.

2. I recognize that there are certain inherent risks associated with all forms of Irish dancing and/or exercises and drills and I assume full responsibility for any injury to myself and (if applicable) my family members, and further release and discharge any teacher, employee, representative or agent of O’Grady Quinlan Academy of Irish Dance, Inc. for any loss or damage arising out of my use or my family’s use of, or presences upon, the facilities used by O’Grady Quinlan Academy of Irish Dance, Inc., whether caused by the fault of myself, my family or other third party. In addition, I recognize that there are certain inherent health risks associated with group activities and I assume full responsibility for any illness that may occur to myself and (if applicable) my family members by participating in such activities, and further I release and discharge any teacher, employee, representative or agent of O’Grady Quinlan Academy of Irish Dance, Inc. for any loss or damage arising out of my use or my family’s use of, or presences upon, the facilities used by O’Grady Quinlan Academy of Irish Dance, Inc., whether caused by the fault of myself, my family or other third party.

3. I agree to indemnify and defend O’Grady Quinlan Academy of Irish Dance, Inc. against all claims, causes of action, damages, judgments, costs or expenses including attorney fees and other litigation costs, which may in any way arise from me or my family’s use of the facilities (Easton or Allentown Studio) used by O’Grady Quinlan Academy of Irish Dance, Inc.

4. Student/staff safety and wellness is our utmost priority. Modifications to our class schedule may occur at any time to protect the health and safety of students and staff.

5. I agree to pay for all damages to the facilities used by O’Grady Quinlan Academy of Irish Dance, Inc. caused by me or my family’s negligent, recklessness or willful actions.

6. Payments made to O'Grady Quinlan Academy of Irish Dance, Inc. are non-refundable for any reason.

I HAVE READ THIS DOCUMENT AND UNDERSTAND IT. I FURTHER UNDERSTAND THAT BY SIGNING THIS RELEASE, I VOLUNTARILY SURRENDER CERTAIN LEGAL RIGHTS.

By clicking my consent below I agree to all the waivers and policies listed.

I have also read and reviewed and agreed to both the OGQ Dancer, Parent, and Social Media Code of Conducts. I agree to hold myself and my family and household members accountable to these codes of conduct as a member of this dance school.

I have read and reviewed the Fundraising and Recital requirements as they apply to my dancer and their level.

I also agree to the tuition structure laid out in the registration documents that pertains to the level of my dancer/dancers. Any late fees and failed payment collections are my responsibility. Dancer resignation terms are laid out in each of these documents and I agree to abide by those terms.

I also agree to hold O'Grady Quinlan harmless to any incident related to the waivers listed. I also note that these waivers are in effect until a new waiver is signed or both parties agree to the waiver revocation in writing.

Dancers under the age of 18:
The O’Grady Quinlan Academy has my permission to use my or my child’s photograph publicly to promote the school. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.

Dancers 18 and older:
The O’Grady Quinlan Academy has my permission to use my photograph publicly to promote the school. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.

In addition, any feisanna/provincial hosted by an An Chomhdhail branch/school will have permission to announce/post names, pictures, and anything else relative to the feis/provincial including but not limited to results, awards, official photography pics on social media for advertising purposes, etc.

Anti-Bullying Policy

O'Grady Quinlan is committed to providing a safe and supportive community that fosters a lifelong love for Irish Dance. In order to guarantee that all of our dancers are able to best pursue their passion, we have implemented a comprehensive anti-bullying policy applicable to our students, dance families, staff, and employees.

We define bullying as any behavior physical, verbal, or written (either electronically or in person), that detrimentally targets another dancer. This behavior is described as bullying if it physically, emotionally, or mentally harms a dancer, targets a dancer for any actual or perceived characteristic, or creates a hostile environment in the dance studio.

REPORTING BULLYING:

We understand that the under reporting of bullying is the biggest hindrance to stopping further harassment. Please email OGQteachers@gmail.com with any concerns.

A member of the OGQ Staff will then contact the alleged bullying party, the alleged victim, teachers, assistants, and parents who may have possibly witnessed the event. If he/she finds evidence that corroborates the report, this will serve as the basis for scheduling a course of action meeting. Any staff member including educators, associates, and administrative personnel are required to intervene if they are made aware of the possibility that bullying is occurring, including but not limited to witnessing bullying. All personnel are required to intervene by filing the report directly. If a parent is the one making the studio aware of the bullying event, the parent is required to take on the primary responsibility of filing a report.

VIOLATIONS:

OGQ will not tolerate bullying in any form: physical, emotional, social or otherwise. As such, we have implemented a Zero Tolerance Bullying Policy. Since our goal is to empower people and embrace individuality, one documented bullying event will have repercussions which may include suspension from classes, performances, competitions or immediate termination of enrollment for egregious violations.

Please note, as per the policy above, OGQ recognizes that bullying does not just occur between students. If a parent or guardian has witnessed bullying or is harassing another student, parent, or OGQ staff member, there will be repercussions. Any penalties above are to be made without any refund for classes missed, competitions or costumes.

At OGQ, respect and compassion are two of our core values. All dancers are entitled to learn and develop their art in a positive, respectful environment. We realize that every situation reported through this form may require a unique follow-up and each case may be handled differently.
