Assumption of Risk: I understand that participating in dance activities involves certain risks, including but not limited to physical injury. I voluntarily assume all risks associated with my child's participation in the dance/movement program.
Release of Liability: In consideration of my child being permitted to participate in the dance/movement program, I, on behalf of my child, hereby release, waive, discharge, and covenant not to sue Precision Dance, its owners, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child, whether caused by the negligence of the organizers or otherwise.
Medical Treatment: I authorize the instructors of Precision Dance to seek medical treatment for my child in the event they are unable to reach me or another emergency contact. I understand that I will be responsible for any medical expenses incurred.
