Tuition payments will be automatically charged on the 15th of each month, for the upcoming month. If you register in the middle of a billing cycle, you may have a payment due right away.

Studio Diva does not carry medical insurance for its students. It is required that all students be covered by their family's own insurance policies. If injury occurs, it is understood that the student's own policy is their only source of reimbursement.

I herby authorize Studio Diva to electronically withdraw funds for my child's monthly tuition, registration fees, or other fees due from the credit card I have listed on file, by the 15th of each month. I understand that I may at anytime change the payment information information on file through my JackRabbit Parent Portal, or by contacting Studio Diva directly. Unpaid tuition accounts will be charged a late fee of $10 on the 16th of each month. Students with unpaid tuition after 14 days will not be permitted to attend class until the balance has been paid.

I acknowledge and consent to allow Studio Diva to use: photos and/or videos of my child/myself in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), our studio's website, or on social media sites (such as the studio's Facebook or Instagram account). I further understand no financial compensation will be for use of these photos/videos.

Registration Fee: For new families, a one-time registration fee of $50 per family is due at the time of registration. Registration will not be considered complete, and class space is not reserved until this fee has been paid. Payment Policy: Tuition is billed a month in advance by the 15th of each month. Balances will be automatically charged to the card on file on your chosen payment date. Unpaid tuition accounts will be charged a late fee of $10 on the 16th of each month. Students with unpaid tuition after 15 days will not be permitted to attend class until the balance has been paid. *Tuition may be paid by the following methods: cash, check, or credit card (We accept MasterCard, Visa, and Discover). *Checks should be made payable to Studio Diva, and include your child's full name in the memo. There is a $10 returned check fee, per check, on all returned checks. *Tuition remains the same regardless of absences. Attendance: Good attendance is imperative, as absences and tardiness can hold back an entire class. Students should be prompt and prepared for class each day. In the event of a class cancellation/studio closing (Due to bad weather, emergencies etc..), there will be a make up day provided to the students. All parents/guardians will receive notice if there is a change in the schedule.
